轉移性大腸直腸癌標靶藥物 6月起獲健保給付

健保自2011年6月1日起，給付轉移性大腸直腸癌患者可以在第一線合併使用血管新生抑制劑標靶藥物進行化學治療，預估每年至少5千名轉移性大腸直腸患者受惠，未來腫瘤有機會獲得控制，讓存活率大幅提高。
近年來，隨著國人飲食西化，經常吃油炸、高油脂及膽固醇食物，蔬果攝取不足，使得罹患大腸直腸癌人數不斷攀升。根據統計，大腸直腸癌已連續3年高居國人十大癌症前三名，國內每年新增1萬多名大腸直腸癌患者，其中僅5%為家族遺傳，大部分與後天因素，例如攝取太多脂肪性食物有關。

大腸直腸癌是隱形殺手，早期沒有明顯症狀，直到出現血便、腹痛、排便習慣改變、便秘或嘔吐時，就醫才發現已是晚期。台北榮總血液腫瘤科主治醫師鄧豪偉表示，「排便不規律是罹癌前兆主要表現之一。」基本上，若腫瘤靠近肛門位置，患者可能較容易經由指診診斷；當腫瘤出現在大腸前端或其他位置，此時易延後診斷。

鄧豪偉說，大腸直腸癌治療複雜，通常第三期前多以開刀處理為第一線治療；但在直腸癌，若腫瘤過大或太接近肛門，可先同步電療、放療後再開刀，至於轉移性大腸直腸癌較棘手，患者多半已無法開刀，只能注射化學藥品治療。不過，健保6月起給付化學治療合併抗血管新生標靶藥，作為癌友第一線治療，患者比過去至少能增加4-6個月存活時間，腫瘤有機會控制、縮小與切除，部分癌友甚至可被治癒，5年內不復發。

鄧豪偉指出，簡單而言，血管新生抑制劑標靶藥物就是把供應腫瘤養分的血管，直接將補給線截斷，讓腫瘤壞死、無法生長，癌細胞不能增生或轉移。基本上，標靶藥物需兩周注射一次，使用劑量依患者體重差異調整，通常每公斤體重應注射5毫克，如果以體重60公斤的成年人來說，每月需注射6支針劑，藥費約11至12萬元。

6月起，血管新生抑制劑標靶藥物納入健保一線治療給付後，第一次申請療程為12周，若患者情況穩定、治療有效、腫瘤沒惡化，第二次申請得再延長12周，健保最多給付24周。

不過，標靶藥物也有副作用，常見症狀包括高血壓、尿蛋白、出血或血栓等，嚴重者可能腸穿孔死亡，但發生機率不高，僅1%至3%。鄧豪偉說，一般而言，高血壓及尿蛋白副作用可用藥物控制，至於出血或血栓能以凝血劑或抗凝劑治療，但鄧豪偉強調，治療過程若出現不明原因腹痛，應立即急診就醫檢查，避免腸穿孔危險。

鄧豪偉呼籲，民眾若有大腸直腸癌家族史者，除每年做糞便潛血檢查外，每兩年應再做大腸鏡檢查；一般人則建議40歲以上最好每年做糞便潛血，才能早期發現問題，即早開刀治療。
