繼高病原性H5N2禽流感成為國內養禽場特有本土性疾病後，昨天出現首宗人類感染H6N1禽流感個案，讓人覺得台灣禽流感疫情還真獨步全球。

H6N1是普遍存在國內家禽間的低病原性禽流感病毒，全球未曾發現人類感染病例，一旦出現感染病患，顯示該病毒在台可能已經變異。

農委會長期疏於監測國內養禽場普遍存在的低病原性禽流感病毒，不是讓H5N2演化成高病原性，就是讓H6N1演化成會感染人類，實難辭其咎。

其實早在一九七二年就在台灣養禽場發現H6N1。當年在淡水鎮竹圍地區靠近淡水河某養鴨場，放養的八百隻三周齡肉鴨染病，多呈現呼吸症狀陸續死亡。由病鴨分離出的H6N1病毒，被認為是淡水河畔一帶的候鳥水禽感染造成，這也是台灣首次發現的家禽禽流感疫情。至今，該病毒已普遍存在國內養禽場，由於病原性極低或幾無病原性，農政單位並未作任何防疫措施。

令人訝異的是，病毒在家禽間經過四十幾年的演化，居然會變成人類致病的病原。筆者認為現時最重要的工作，是盡快完成基因排序，以確定病毒是否變種？何以會感染人類？該罹病個案會不會只是冰山一角？盼望防疫、農政單位要未雨綢繆，謀求對策。

二○○七年國內某學術單位研究報告指出，台灣發現的H6N1病毒株，與中國、香港的分離株不同，為台灣特有的本土株。報告也指出，台灣養禽場的H6N1病毒株，雖屬低病原性，但動物接種實驗發現，某些直接採自雞隻的病毒株，可以在小白鼠身上增殖，顯示這些病毒具有跨物種感染的潛在危險。可惜研究報告未受官方重視。

根據國衛院資料，去年初台灣五個養雞場爆發的H5N2禽流感疫情為高病原性，分析病毒的基因序列發現，近年流行的H5N2病毒株，其八段基因極為特殊，有二段基因（HA和NA）來自美洲的H5N2病毒，其餘六段基因則來自台灣本土的H6N1病毒。這種基因序列特殊的H5N2病毒株，目前只在台灣偵測到，並且已成為台灣在地化病毒。農委會縱容低病原性禽流感病毒在養禽場間流竄，不作撲殺等防疫措施，這些養禽場會不會淪為新興禽流感病毒的發源地？

台灣出現全球首例H6N1禽流感病患，必受各國衛生機構矚目。農委會應亡羊補牢，加強養禽場的監測檢驗，必要時應清場撲殺補償，防範其失控造成跨國流行，否則恐對台灣的國際形象極為不利。
