市府採購ＡＥＤ 救胖子不夠力？

〔記者郭安家、邱宜君／台北報導〕台北市衛生局預計三年內採購一千組自動體外心臟電擊器（ＡＥＤ），但有醫療器材業者向本報投訴，指衛生局標案設計讓保全業者與ＡＥＤ代理商結盟，成本計算下，最後卻引進較便宜的二百焦耳半自動ＡＥＤ，而非總統馬英九座車配備的高階三六○焦耳全自動ＡＥＤ，對此，衛生局表示二百焦耳已符合民眾使用需求。

醫師：效益一樣好

台北市醫護管理處長劉越萍說，標案沒有說ＡＥＤ要結合保全業者，但有要求投標廠商需有人廿四小時待命維修機器，要有人力支應急救，ＡＥＤ廠商可以找任何團體合作。

對於二百焦耳ＡＥＤ無法救治大胖子疑慮，台大醫學院急診醫學科教授陳文鐘表示，在臨床上，不論哪一種焦耳數的電擊器，臨床效益「都一樣好」，但沒有實證能指出是否三六○焦耳比二百焦耳更有效。

台大醫院急診醫學部主治醫師馬惠明說，焦耳數是一個固定的能量，但ＡＥＤ給每個人的電流量不固定；隨人體胖瘦、水分多寡不同，電流通過會面臨不同程度阻抗，ＡＥＤ貼片貼上去之後，機器會馬上偵測電阻，再依每個人不同的阻抗給予適當的電流量，不會有電流量不足的問題。

但業者也同樣提出竹科生物醫學園區訊息區一篇「體外去顫器風險摘要」，翻譯自美上股份有限公司（MedSun）民國八十八年的ＡＥＤ不良案例彙整報告，指美國兩年間通報六十四起不良案件，其中ＡＥＤ二百焦耳功率不足而需使用三六○焦耳達九例、佔十四％。他說，若衛生局不正視問題，未來恐難避免發生憾事。
