【魏斌、周昭平╱高雄報導】曾獲「總統教育獎」的生命鬥士張恆鈞，多年前便為胃食道逆流所苦，去年九月遭健保局停止給付該症用藥，除非重做上消化道內視鏡檢查，但張恆鈞早已氣切、全身癱瘓，無法承受這項檢查，張母泣訴：「不懂健保局為何非要孩子冒生命危險才肯給藥？」健保局回應，會請醫院協助專案申請處理。

罹患罕見疾病「裘馨氏肌肉萎縮症」的張恆鈞（二十六歲），四歲發病後多次在鬼門關徘徊，高中畢業那年，他被從加護病房推進大學指考考場，雖考上高雄師範大學數學系，但因病重氣切，從此僅能靠鼻胃管進食，二十四小時佩掛呼吸器求生。

月自付千元藥費
張母陳台秀說，恆鈞去年九月感染送醫，「因長期服用胃食道逆流藥物，被健保局抽中審查，須重新進行內視鏡檢查才能給藥。」她想起恆鈞十五歲時，第一次做內視鏡檢查，「麻醉後在診療台上被翻來翻去，幾乎送命！」不敢再讓恆鈞做檢查，沒想到去年十月起，健保局不再給付，每月得自付一千餘元胃食道逆流用藥。
醫院提專案申請
陳台秀說，罕見疾病基金會獲悉此事後，要協助支助藥費，但被她拒絕，「因為這一千塊，省一點就有！」且她覺得恆鈞已得到社會太多關愛，家人也不想張揚此事，自付四個月藥費後，才決定公開，「因為很怕有其他類似患者，既無法檢查，家屬也付不出錢，只好忍痛讓患者承受更大痛苦。」
健保局高屏業務組發言人林淑華說，依藥品給付規定，胃食道逆流須有近四個月內上消化道內視鏡檢查或X光攝影才可給藥，之前未抽審到張恆鈞的用藥狀況。不過，患者可透過醫院申請，待送交專業醫師審查通過，再以專案方式、免做內視鏡檢查情況下，以健保給付使用該項藥物。
高雄市立小港醫院表示，當初是院內藥品使用規範管理系統預警，醫師只得依健保局公告的藥品使用規定詢問家屬，但家屬表明無法進行胃鏡檢查，礙於健保局規定，才請家屬自費使用該項藥物；不過，張的案例特殊，院方已向健保局詢問，並準備提出相關資料進行專案申請。

服鋇劑食道攝影
阮綜合醫院消化系內科主治醫師孫盟舜說，患者若不適合進行內視鏡檢查，可服用鋇劑進行食道攝影，或選擇精密的膠囊內視鏡，但若是特殊狀況的病患仍需進一步評估。

胃食道逆流小檔案

★併發症：食道黏膜發炎，恐致惡性病變、食道癌
