本來手機好一陣子不再被媒體記者的來電打到沒電，怎麼昨天開刀開到晚上，居然有三家電視台輪番來電，大概是因為某人開的課選修人數過少，然後又要扯到我頭上，最好是跟我有關啦！總之，今天早上用餐的時候，母親跟我提到新聞報導有病人告胡瑞恆醫師，嗯，我今天很忙，請記者不要打給我，我想說的都在這裡。

沒有看過病歷，不能評論，不過我們就把新聞裡面得到的資訊拿出來談吧。某人一直在做健康檢查，連續幾年都看到有息肉，因此跟醫師討論，最後決定切除，然後切下來的膽囊沒有息肉，所以決定告主刀的醫師。這樣的認知有錯嗎？如果事實經過不是如此，那就不用討論了。

看完這段故事，只能感嘆「人心不古」啊！以前開刀，開完跟病人說是良性的，病人就鬆了一口氣「謝天謝地」，現在呢？「居然是良性的？告死你！」哈哈哈，你問我為什麼不想當外科醫師？我反到要問你誰願意當外科醫師！

醫療常規上面，膽囊疾病的診斷本來就是腹部超音波最敏感有效，只有在腹部超音波懷疑是惡性的情況下，需要接受其他檢查來確認是否有轉移的可能。既然可以「發現好幾年」都沒有處理，可見一開始就認為是良性的，本來就可以不用開刀。不過我們可以想像病人在門診追問醫師的畫面--「那個膽囊息肉有沒有變大」、「會不會是惡性的」、「不開刀一直追蹤可以嗎，會不會有一天轉移出去」......自己想想看，如果醫師說你膽囊長一個息肉，你會不會有這些疑問。而膽囊息肉的外科適應症裡面，本來就有一項是「病人意願」，如果病人有膽囊息肉而同時表達希望切除膽囊的強烈要求，醫師把膽囊拿掉是合情合理的，不然為了一個息肉要一直門診追蹤，整天疑神疑鬼會不會是惡性的，這很累耶！

手術前一定會說明手術風險，如果病人是高知識份子說明會更清楚，有些特殊行業，像是記者、法官、律師、醫師等等常常說明的內容等於是在跟醫學生上課一樣，原因不用多做解釋吧。我當總醫師的時候，術前說明一定會提到術後腹瀉的可能，當然，有的人拉兩個禮拜就沒事了，有的人會拉一兩個月，不過人的身體會自動適應環境，我沒碰過拉肚子超過半年的病人，或許是我經驗太少啦，你要問我可不可能拉一輩子，我還真沒辦法說不可能，因為醫療是這樣的一門學問，如果醫療的不確定性不存在，我們找電腦機器人看診就好啦。

「僅憑健檢報告就動刀，病人認為太離譜」，如果說健檢報告不能當做臨床依據，那我們還做什麼健康檢查？每次在門診的時候碰到病人說在某家醫院做電腦斷層發現有腫瘤，建議開刀，就跑來我們醫院「希望接受更完整的檢查」，醫療資源的浪費是醫師的錯嗎？這些逛醫院的病人怎麼沒人管啊！別的醫院的電腦斷層是瑕疵品嗎？都做過電腦斷層了，還想來再做一次，幹什麼？如果結果相同，你浪費醫療資源，健保核刪又扣不到你的錢，如果結果不同，下一步呢？再做一次？

之前某個產科新聞，產前檢查都沒發現畸型，結果生出的小孩有異常，病人家屬當然告醫師啦。問題是目前醫界沒有一項產前檢查對疾病的偵測率是百分之百的，既然檢查的偽陰性機率不為零，就是說「報告正常實際異常」的狀況一定會有機會發生，那醫師就一定要來承擔目前科技的不完美嗎？為何不是去告超音波的儀器公司啊？

新聞裡面有這麼一段文字：「連懂法律的人都遭此對待，絕對告到底。」我只能說，連高知識份子都是這種態度，連這麼好的老師都會遭此事端，我真的不知道我有什麼理由要繼續當外科醫師啊！

你想問我胡醫師是不是好醫師嗎？這麼說好了，如果有一天我得了肝癌或是膽囊癌，而胡醫師願意幫我動刀的話，手術前一晚我會睡的很安穩。

超級好笑的，當我準備離開外科的時候，我發現我不需要再跟別人解釋了，只要把最近的新聞標題唸一遍，大家都很能體會我的感覺了XD
