Q：這幾年台灣兒科醫師大量流失，主要原因是什麼？
A：我在小兒科任職10多年。以發展來看，10幾年前，我讀醫學院時，小兒科非常熱門。我第一年要申請進入醫學中心小兒科時，錄取5位，就有60位來報考，特別是87、88年，前後幾批都出現小兒科醫生過剩情況。在醫院工作幾年，我身邊一些兒科醫生都自己開診所，後來，隨著生產率降低，診所一家一家關，像我兩個同學也都把診所收起來。

少子化導致醫學院的學生不選小兒科，醫院也招不到兒科醫師。我們今年要錄取5位，還招不滿，有資歷的醫師都紛紛轉科別，就算健保局最近提高兒科申請健保費的優惠，效果仍然有限，許多醫生還是想走。像我們班當初有15個小兒科醫生，現在差不多有一半已經轉調，而選醫美當做轉行指標佔多數。其實重點就在醫院經營的好壞。
一般來講，經營的好，醫院通常是擴編，但像我待的這家醫院，病床逐年縮減，按比率，目前大概縮了十分之一。病床數縮減，主治醫師的缺就遞減。我們醫院的狀況是，小兒科遇缺不補，如此一來，有心的醫生要往上升遷的機會更難。有的醫生覺得沒有發展空間，不願意等，當然會想離開。升遷對醫生來講，很重要，多半醫生一定都希望能達到主治醫生的位置。

考慮赴中國發展
醫學院就屬內、外、婦、兒四科風險最大，醫生所承擔的壓力也高。我當住院醫師時，每天工作12個鐘頭算正常。最高紀錄是加上值班，連續工作70幾個鐘頭，都沒下班，自己找空檔吃飯、睡覺，比水泥工還不如，不過，這是這行必經過程。所以，住院醫生大概能撐3到5年，無法長期，辛苦過後，又升不上去，當然想走。

Q：面對龐大壓力如何排除？
A：小兒科的病患本來就比較不好照顧，新生兒只要存有先天性或代謝異常等疾病風險，由於這些疾病變化非常快，有時一個判斷錯誤就會危及生命。像這種死亡率較高的科別，壓力很大，糾紛也多，大人的反應通常最激烈。
現在病人更了解自己的權益，醫病關係比以前緊張，所以很多醫生在這等情況下，第一思考到的，會是有沒有被告的可能性？兒科醫生一旦碰上醫療糾紛，通常會被家長咬著不放。家屬永遠認為是醫生判斷不夠精確，處理不好。這對醫生來講，是很大的壓力。我有時下班臉都很臭，脾氣也差，家人最常遭殃。

Q：你會想轉科別嗎？
A：我目前有在評估，當然年紀也愈來愈大，沒辦法讓醫院這樣操。現在有些台灣醫生也在觀望大陸那一塊，在對岸，小兒科醫生相對很缺，醫生素質也不如台灣。我最近常跟同學說，既然台灣環境不適合，到對岸發展或許不錯。
