新生兒百日咳 臉發黑停呼吸

〔中央社〕一名出生30天左右的嬰兒，因出現臉部發黑、血氧下降、單日多次呼吸中止等症狀，醫師高度懷疑感染百日咳。醫師今天說，新生兒感染百日咳的症狀較不典型，呼籲家長注意。

馬偕醫院小兒感染科醫師邱南昌下午在記者會表示，日前收治一名出生30天左右的小嬰兒，因出現臉部發黑、血氧下降、抽筋等現象，轉至加護病房治療，後來小嬰兒白血球增加、1天發生多次嘔吐及呼吸中止等症狀；院方高度懷疑感染百日咳，進行隔離治療。

邱南昌說，新生兒缺乏抵抗力，且肺部功能不成熟，感染百日咳後，無法表現出哮咳、持續性陣咳等典型的百日咳症狀，而是出現例如臉色轉黑、昏厥、呼吸停止等症狀，或類似呼吸道感染、敗血症、心臟病的臨床現象。

他說，若沒有即時診斷，就會延誤投藥時間，導致死亡或腦、肺部功能缺損等嚴重的後果，因此，家長及照顧者要特別留意。

另外，社團法人中華熊媽媽保母公益協進會針對300名平均年齡40歲以上，且執業經驗超過6年的有照保母調查顯示，有6成保母根本不知道自己體內沒有百日咳抗體，也沒有留意今年台灣的百日咳疫情已有升溫的趨勢。

進一步分析發現，超過4成的保母正托育未滿1歲的新生兒，而未打完百日咳疫苗基礎劑的1歲內嬰兒，正是感染風險及致死率最高且病情最嚴重的族群，顯示針對保母從業人員宣導補打成人百日咳疫苗有其急迫性。

邱南昌說，多數成人都沒有百日咳抗體，很容易成為感染或散布百日咳病菌的帶原者，若剛好有接觸小嬰兒的機會，可能引發嬰幼兒感染危機。

中華民國周產期醫學會理事長許德耀表示，由於台灣家長托嬰的情況普遍，更應針對長時間接觸嬰兒的保母，進行高危險性的傳染病疫苗注射宣導，並重視保母的疫苗施打紀錄，以避免寶寶成為百日咳的受害者。
