藥師太忙 平均只給病患諮詢35秒

民眾到醫院看病領藥時，多會有一個共同的經驗，就是要先開口問藥師，藥師才回答，根據藥事品質改革協會（簡稱藥改會）調查發現，有8成的藥師是因為病人主動詢問如何使用藥品，才開口說話，且藥師提供諮詢的時間平均僅有35秒鐘，與實際測量要充分完成「用藥說明」的平均時間2分22秒，實在是「差很大」。


藥改會於今年一月針對國內20家醫學中心藥師交付藥劑諮詢狀況進行實地調查，結果發現，6.3%的藥師沒有確認病人姓名，即交付藥劑，嚴重違反調劑的規定，有誤發、誤用藥品的風險，且僅有2成5的藥師會向病人說明藥品禁忌症、副作用及服用藥品注意事項。


藥改會理事長余萬能表示，會造成這樣的結果，並非藥師本身的問題，此次實地調查也發現，醫學中心藥師一天需服務450名病患，造成超過2成5的藥師無法提供病人領藥時的用藥諮詢。


余萬能進一步指出，美國的合理調劑量為每日30張，日本為40張，我國健保局則訂門診為100張，衛生署新制醫院評鑑門、急診則訂為70張到80張。


事實上，根據成大臨床藥學研究所高雅慧教授的研究結果指出，藥師調劑一張處方箋所需時間至少7分32秒，其中用藥教育約需2分9秒，因此推估一位藥師每日調劑處方的合理上限值為64張。


研究顯示，超量的藥品調劑工作，使得藥師沒有能力依法定事項充分說明，導致民眾可能錯誤用藥的機率大增，藥師也自處於違反規範義務的危機環境。


藥改會呼籲，衛生署必須建立有效的「合理調劑量」制度，並將藥事作業納入醫院評鑑評分的必要項目，才能確保民眾最基本的用藥安全及藥事服務品質，而在未納入規範前，建議健保支付制度，應採藥費與藥事服務費同時斷頭方式，讓醫院無法用藥費價差彌補藥事服務費的虧損，才能讓醫院不再要求藥師嚴重超量工作。


對於藥改會的訴求，衛生署及健保局均表示，可以再討論研議。
