打石膏、五十肩…當心關節沾黏

案例一：讀小學六年級的廷廷，上體育課拉單槓的時候跌落在地，送到醫院診斷手肘骨折，上了石膏。原以為拆完石膏，馬上可以打球，沒想到受傷的手肘已完全僵硬，連彎都不能彎。


案例二：張太太是一位勤勞顧家的主婦，日前因為大掃除，不小心拉傷了肩膀。本以為休息幾天就會慢慢恢復，沒想到手卻越來越舉不高，復健科醫師告知，她得的是典型的「五十肩」。


◎什麼是關節沾黏？

關節是兩個骨頭相交接的地方，作用就好像樞紐一樣，讓身體能有更大角度的活動。身體的關節因為不同的原因，失去了原來可以自由活動的角度，就叫做關節沾黏。


◎關節為什麼會沾黏？

大部分的關節沾黏是因為「缺乏活動」所造成，最典型的例子就是五十肩。另外一種常見的情形是「打石膏」，骨折以後，不管有沒有開刀，醫師會用打石膏的方式固定。但也就是因為石膏固定以後，關節就不能活動，等到骨頭長好了，關節也硬掉了。其他像關節炎、脊椎神經受傷、中風、腦傷，或是長期臥床的病人，也都會發生關節沾黏，甚至關節變形。


至於長期臥病在床，又沒有人幫忙做復健的病人，就可能全身的關節都發生沾黏。


◎關節沾黏會如何？

關節沾黏時，身體就不能自由順暢的活動，有時候會連日常生活最基本的吃飯、走路、梳頭髮都會有障礙。關節沾黏會有明顯的疼痛。嚴重時，連休息不動也會隱隱作痛。如果是長期臥床的病人，還可能導致褥瘡，造成感染。


◎如何診斷？

只要是有經驗的復健科醫師，會先觀察病人能自主活動的角度，就是所謂的「主動」關節活動度，再加上醫師幫病人活動的「被動」關節活動度和手感，就可以知道關節是不是有沾黏的現象。


◎如何復健？

簡單地說，就是把已經沾黏變硬的關節拉鬆。復健可以分成兩部分：


●醫院裡的復健治療：復健科醫師會依據病人關節沾黏的程度，用不同的方法（儀器、徒手）來拉鬆關節。在治療以前還會用熱敷和電療的方式，放鬆關節附近的組織。


復健治療師在「拉」關節的時候，病人常常會痛得哇哇大叫，甚至淚流滿面。我常常告訴病人：「痛苦是必經的過程，你一定要撐下去，因為這是『好的痛』，千萬不要怕痛！」


●病患在家裡的運動：如果只希望靠在醫院的復健就想痊癒，那就會大失所望了，因為病人自己在家裡的運動才是康復的關鍵！復健科醫師和治療師在治療的時候，一定會教病人許多自己可以在家裡做的運動，主要的目的就是要藉著這些運動來維持復健的效果。


◎是否一定要開刀？

有一小部份的人真的會因為關節的沾黏而開刀，但都是屬於比較嚴重的病患，或者是復健治療的效果不佳才會考慮。開刀主要是把關節裡面沾黏的部份清一清，然後再把整個關節拉一拉。


要特別強調的是：開刀只是治療過程中的一個階段。開完刀後，更要趁著關節比較鬆弛的黃金時期，加上更積極的復健治療及運動，否則關節還是會再硬回來的。
