腸癌腫瘤位置不同 症狀有所差異

大腸癌成為國人發生人數最多的癌症，醫師表示，儘管腸癌初期症狀不明顯，但腫瘤生長位置不同，表現症狀還是會有所差異。「右側腸癌」早中期最難察覺；「左側腸癌」因腫瘤阻塞腸道，容易腸絞痛、便秘和腹瀉；「直腸癌」則容易出現血便和頻便，呼籲民眾，五十歲開始應該定期篩檢，高危險族群則從四十歲起、提早十年就應該開始檢查。　(徐韻翔報導)


　　大腸癌一般初期症狀不明顯，等到出現不適時，通常已非早期，而腫瘤位置不同，所表現出來的症狀也會有所差異。


　　台中榮民總醫院大腸直腸外科主任王輝明表示，腸道一般分為右大腸、左大腸與直腸，右側的早期大腸癌，一點症狀都沒有，發現時會較晚期，他解釋，當腫瘤在右側時，經過這個位置的糞便是水便的狀態，即使有腫瘤在腸道，也不太會堵塞，一旦有堵住症狀時，腫瘤恐怕已經很大了，此外，右側大腸癌即使有潛血反應，血和糞便混在一起，等糞便運送到肛門口要排出時，血跡早就不明顯了，因此，右側腸癌，在早中期以前，很難發現，而據他自己的門診統計，檢出罹癌的患者，有四分之一的人癌細胞已經轉移。


而左側大腸癌相較於右側，則比較容易自覺發現，王輝明解釋，由於糞便在大腸裡水份漸漸被吸收之後，糞便運送到左側大腸時，已經呈現較固體狀，有腫瘤時，便容易堵塞，引起絞痛、便秘等反應，有時候糞便堵塞過多壓力過大衝到直腸時，又會造成腹瀉，因此大便習慣改變是左側大腸癌最典型的症狀。


至於直腸部分，由於距離肛門口最近，只有十五公分，醫師表示，如果有腫瘤，會容易刺激直腸收縮，引起血便與頻便的反應。


醫師提醒民眾，要預防腸癌，一定要重視篩檢，一般五十歲起，應開始定期接受大便潛血檢查，陽性反應則進一步透過腸鏡等檢查確認病況，高危險群有遺傳史的族群，則提前十年，從四十歲起開始定期篩檢。
