破除乳頭迷思 少奶奶勇敢挺胸向前

門診中，來了一名年約40歲的初診女病患，她因罹患乳癌剛做完部分乳房切除術，必須接受術後的放射治療。

患者表示，她剛發現乳房有腫瘤時，就去掛別家醫院的某位外科醫師的門診，醫師建議要馬上開刀，並且和她商量可能的開刀方式，當時就表明要保留乳頭。她覺得，雖然她可以接受乳房部分切除的手術以及術後追加放射治療，但是她無法接受乳頭被切除，因為沒有乳頭的乳房是沒有功能的，是殘障的。

但是手術當中，因為腫瘤太靠近乳頭，所以外科醫師就走出手術房和家屬商議，是否切除乳頭，以免腫瘤已經侵犯乳頭，於是家屬（後來我才知道：其實是她的前夫，但是外科醫師當時並不知道）簽了名，醫師再度進入手術房中，完成連乳頭一起切除的乳房部分切除的手術。

術後她醒來之後，如同遭到青天霹靂一般，完全無法接受自己殘缺的身體，開始陷入無盡的悲傷，每天以淚洗面，更無法一覺到天亮！而後病理報告出來了，更顯示乳頭並未受到癌細胞的侵犯，因而她的內心開始責怪醫師違背之前與她的承諾。

◎聽了她的遭遇，我首先向她解釋，並替外科醫師緩頰：

●因為腫瘤太靠近乳頭，所以乳頭有被癌細胞侵犯的可能性，因此外科醫師是站在希望完全切除腫瘤，以增加病患治療成功率的角度上，所做的決定。而這個決定無可避免地與美觀及自我形象的完整性起了衝突。

●切除乳頭是經過家屬的同意，所以在法律上，外科醫師並沒有過失。

剛開始，她每天下午2點半下班以後，就來接受放射治療。可是門診時，都可以看出她的憂鬱，我嘗試去了解她內心的困境，並且提供一些方法，例如整形、靜坐或是任何的宗教信仰、介紹少奶奶癌友會的朋友給她認識等等，也希望給她一點鼓勵。

可是終究不敵她內心對乳頭的執著，最後她中斷放射治療，因為她既然不能接受目前的身體，又何必做放射治療，去降低癌症的復發率呢？甚至她想早一點死去！

她還會感覺到傷口偶爾的抽痛，好像在提醒她內心的創傷，尤其天氣變化的時候，胸口的抽痛還會持續一整天，牽制她的心念完全縈繞在這永無止境的深痛裡。

不經意地看著路邊身體殘障的人依然忙碌地過著他們的人生，她雖然敬佩他們，卻承認自己無法做到那麼勇敢！負向的思考一直縈繞在心中。

所幸現在她覺得就算過了幾年，她已經調整好心態，接受她術後的乳房，只是不知道未來命運還會不會再開另外一個玩笑來試煉她？無限的沉思伴隨僅存的夕陽餘暉，我只能輕輕地感嘆自己能做的太少吧！
