退燒藥吃多 幼兒差點爆肝

幼兒發燒不能一味退燒。醫師發現，有家長給幼兒吃退燒藥，短時間內沒有明顯退燒，又接著用藥，一天下來，用藥劑量超過正常的兩倍，幼兒送醫時肝功能指數已升高。另外，有4成的家長以冰枕或溫水洗澡為幼兒降溫，忽略階段處理的重要性，造成幼兒失溫，發燒越拖越久。


書田診所小兒科主任陳永綺昨天指出，體溫超過38度即是發燒，是細菌或病毒侵入人體時，免疫系統對抗的發炎反應，39度以上的高溫會帶來頭暈、痠痛等不適，服用退燒藥只能讓體溫降低0.5至1.5度，減緩身體不適，不可能急速降至正常體溫，用藥不能超過一天正常劑量


陳永綺表示，曾有病例服用退燒藥後沒有退燒，家長又讓小孩吃藥、塞肛門退燒劑，一天喝120西西的藥水，明顯超量、肝功能指數升高，必須緊急治療，讓肝功能恢復正常。


陳永綺利用門診對家長進行問卷調查，總計120份問卷，調查發現，幼兒發燒時，57％的家長會給退燒藥、28％會使用冰枕、13％會用溫水幫小朋友洗澡降溫。


陳永綺解釋，為幼兒降溫除使用退燒藥外，還應留意不同階段的處理原則，第一階段幼兒會發抖、手腳冰冷，這時應以保暖為主，若讓幼兒睡冰枕或以洗溫水澡降溫，反而會持續發冷，延緩病程發展，增加心肺負擔。若手腳發熱、心跳及呼吸加速，即進入第二階段，這時可讓幼兒洗溫水澡、睡冰枕，以輔助散熱。


陳永綺強調，處理發燒並非僅觀察體溫，同時要注意幼兒活動情形，若服藥後未惡化，活動力不錯，家長不需過度擔心。不過，若一天之內退燒、發燒不斷循環，伴隨頭痛、全身不舒服、哭鬧不停、怕光、發抖、嘴唇發紫、煩躁不安等狀況，即需就醫。
