安全準備金 累計破二千億

〔記者林惠琴／台北報導〕補充保費真是大補！在補充保費挹注下，健保財務空前穩定，至今年八月底安全準備金已達二○八五．五三億元，足以因應四．六九個月的支出，高於法定一至三個月標準，原本考量人口老化，健保會委員認為費率不動為佳，但眼見馬政府一直在打健保的主意，直說不如調降一般、補充保費費率以回饋民眾，最快會在本月討論。


馬政府動歪腦筋 圖減負擔

補充保費從二○一三年起開收，當年就收取四○三．三一億元，去年更達四六六．二三億元，今年一至四月也有一五二．三億元，讓長期不穩定的健保財務揮別陰霾，二○一三年底累計結餘已達七四六億元，去年再增為一二六○億元，至今年八月底更有二○八五．五三億元。

然而據健保最新推估資料，最快二○一七年入不敷出，若當年度以低、高推估成長率四．○二五％、計算五．八％計算，赤字達八十二億元、一四七億元，隔年更上看二九二億元、三六一億元。醫改會研究員沈珮涵認為，政府應該趁健保財務良好時，改革爭議制度，並改善基層血汗醫護、新藥新科技給付、急難或偏鄉醫療等問題，以利永續經營。

健保會委員：不如調降回饋

但民間監督健保聯盟發言人滕西華痛批，對於民眾好的事情，政府通通不想做，反而思考如何減少政府負擔，既然無法廢除備受批評的補充保費，也無法做出讓民眾有感的政策，調降健保費率回饋於民不失為一個選項。

兼任健保會委員的消基會名譽董事長謝天仁也表示，若政府要對健保「上下其手」，未來支出不足再轉嫁到民眾身上，相關委員認為倒不如先調降保險費率，以免老百姓有「沒錢就喊漲，有錢卻不降」的負面感受。

衛福部長蔣丙煌上任後，曾經宣稱在二○一七年底前，健保費率不會動，但二○一三年健保會決議，安全準備金達三個月就必須討論是否調整費率，謝天仁也認為，二代健保最重要的精神是收支連動，費率須按財務狀況調整，既然財務穩定，就要回饋於民，「不是部長一個人說了算」。

最快本月討論是否調整費率

兼任健保會委員的工業總會常務理事何語也指出，九月下旬健保已完成明年度總額協商，依照《全民健康保險法》規定，總額協商後一個月內，必須依據收支狀況評估費率是否調整，因此一定會討論。

不過，健保署財務組研究員唐薏文說，雖然健保安全準備金是開辦以來最高紀錄，但補充保費收入多寡與景氣相關，現在收得好，不代表可以一直好下去。衛福部社會保險司長曲同光則指出，健保安全準備金高於法定水準三個月或低於一個月，就須全面性檢討費率，因此接下來會討論，但未必調整。

今日上午在黨團幹部會議中，毛治國院長提出世界經濟情況很糟糕以及我國第三季ＧＤＰ為負成長等資訊，同時間，衛福部社保司曲同光司長卻表示明年醫界健保總額成長率將是十年來最高(尚未定案)，並希望醫界要共體時艱！

由於財政部張盛和部長於立法院財政委員會表示不反對減稅救經濟，立法院、媒體及部分朝野立委基於健保安全準備金高達2085億元，也陸續要求衛福部考量調降健保費率之意見，同時，今日自由時報也報載全民健保委員會部分付費者委員甚至將提案調降一般健保費費率。

綜合以上各項意見，清泉認為，即然健保安全準備金主要來自補充保險費之挹注，且足以因應4.5個月的健保支出，符合健保法第78條所定健保安全準備金總額以相當於最近精算1個月至3個月之保險給付支出為原則之規定，則在盱衡我國經濟現況，以及不衝擊醫療費用支出及不影響長期健保財務收支平衡與減少各界過度關心健保安全準備金等前提下，或許可以朝向將補充保險費從2％降為1.5％之方向研議？但無論如何，仍尊重全民健保會依據健保法5條賦予之審議權限及該會最終研議決議！

清泉雖認為現階段健保以維持現行費率為宜，但礙於立法院運作模式，尚需兼顧各種面向壓力而提出部分建議，但一切仍遵守相關體制規範；此外，清泉來自醫界，也是醫界一份子，有關健保費率調整之意見，當會以不衝擊醫療費用支出及不影響財務平衡為優先考量，請醫界同儕無須擔慮。
