每年健檢還罹癌 醫師：大腸鏡有死角

前總統李登輝每年做健檢，為何腫瘤長到三點五公分才發現？醫師表示，大腸鏡有死角，正子攝影（ＰＥＴ）亦有偽陰性，李登輝過去有大腸息肉，應每年做大腸鏡檢查較安全。

「應該九十八年（大腸鏡檢查）就看得到（腫瘤）。」台灣腸癌病友協會理事長王輝明以日本研究為例，從小息肉到癌症成形大約十五年，癌症超過兩公分就算大。幸好，李登輝幾乎每年都做大腸鏡檢查，加上老人腫瘤生長速度較慢，他研判癌症期別應為早期，癌細胞轉移機率不大。

台大醫院大腸直腸外科主任梁金銅從北榮釋出訊息研判，李登輝最有可能是第二至三期大腸癌，八成機率為第二期。新光正子斷層造影中心主任陳遠光指出，ＰＥＴ有偽陰性及偽陽性問題，大腸癌偽陰性雖僅百分之三至五，腫瘤仍可能被遺漏。他強調，李曾有大腸息肉病史，需每年做大腸鏡檢查。

「ＰＥＴ絕對不準。」王輝明說，通常癌細胞會吸收較多葡萄糖，ＰＥＴ根據葡萄糖代謝，了解細胞是否有不正常狀況，但無法確定是否罹癌。

不只如此，王輝明說，「大腸鏡也有死角」。他解釋，大腸鏡操作通常是從肛門一路進到盲腸，之後再往後退檢查影像，有時會因大腸內膜沾黏、影像死角，或腫瘤太小而遺漏。他強調，「兩年內腫瘤長這麼大較少見」，研判可能是兩年前腺癌太小，尚未超過一公分，不容易發現。

另外，以李前總統近九十高齡，究竟該採傳統剖腹手術，或是腹腔鏡手術？臨床醫師有兩派看法，王輝明說，若擔心腫瘤擴散，以傳統手術進行剖腹探查，無可厚非；但梁金銅強調，李登輝年事已高，又有心臟病及糖尿病史，在腹部開十公分傷口，不免讓人擔心傷口癒合狀況。
