止痛止咳藥若含可待因 孩童不宜

英國研究發現，使用普遍的含可待因（Codeine）止痛藥與止咳藥可能不安全、無效且易成癮，不該讓孩童服用。


英國每日郵報報導，藥物專家呼籲檢討可待因應否用於任何藥物，甚至考慮禁用。


可待因是一種能夠讓人放鬆的麻醉藥，英國每年賣出兩千七百多萬盒含有該成分的止痛成藥，其中，包括知名品牌Nurofen Plus、Solpadeine Max、普拿疼Ultra（Panadol Ultra）、Syndol與普除痛錠（Paramol）。有許多人對這些成藥上癮，有的人一天要吃七十顆止痛藥。


根據英國醫療藥品暨保健產品管理局（MHRA）先前規定，以可待因為主的咳嗽糖漿對十八歲以下孩童風險大過益處，應限於成人使用。該局表示，「幾乎沒有證據」顯示含可待因成分的止咳藥可幫助孩童止咳。


台灣的兒科醫師表示，小孩止咳本來就極少使用含可待因藥物，除了因藥物副作用，效用也不大；藥師也建議，若家長有疑慮，現已有不少止咳藥已不含可待因。


可待因最常用的是止咳糖漿。衛生署指出，長期使用可待因，可能出現幻覺、妄想，還可能成癮，一旦停用，會出現全身痠痛、流鼻涕、打呵欠及失眠等症狀。


由於此藥透過肝臟代謝，衛生署指出，除了肝腎功能不全者要小心、減量使用外，嬰幼兒肝臟系統尚未發育完全，使用此成分藥物較容易引發惡心、嘔吐、嗜睡等不良反應。


台大醫院小兒科主治醫師呂立說，兒科醫師極少開給孩子可待因藥物，若孩子出現慢性咳嗽，主因多是過敏、氣喘，或是長期抽二手菸等狀況，應盡量找出慢性咳嗽原因，才能對症治療，可待因藥物對過敏引發的慢性咳嗽，只能緩解症狀，沒有實際效果。


林口長庚醫院藥劑部主任鄧新棠說，多年前，衛生署就不建議兩歲以下幼兒使用含可待因藥物，現在有不少止咳藥已不使用可待因，效果也不錯，若家長有疑慮，可選擇不含可待因止咳糖漿。
