乳房疼痛 乳癌可能性不高

乳房莫名痛起來，有些婦女會以為得了乳癌而驚恐萬分！對此，台北醫學大學附設醫院一般外科主治醫師洪進昇表示，有些婦女會因為乳房莫名疼痛，進而懷疑是不是罹患乳癌或是出現乳癌初期症狀，事實上罹患乳癌的可能性並不高，但若同時有觸摸到硬塊或腫塊者，就需進一步檢查以便確認。

洪醫師指出，乳房疼痛可分周期性和非周期性兩種，周期性疼痛大多和月經周期有相關，常在月經周期前七到十天就開始，月經開始後疼痛就會逐漸緩解，而疼痛原因多半與荷爾蒙有關。當婦女月經快來臨前，雌激素和黃體素分泌將產生變化，然後刺激乳腺組織，壓迫神經，此時往往產生腫脹甚至疼痛的感覺。

洪進昇醫師進一步指出，根據人體神經分佈範圍來看，一般疼痛會出現在女性的乳房、乳暈周圍，或是疼痛轉移至腋下、手臂內側。不過，疼痛大多是陣發性抽痛，局部壓迫時會較痛，但不會隨姿勢或時間而有明顯改變，但單側或雙側疼痛都有可能，會因人而異。

非周期性的乳房疼痛，包含的因素很廣，像是良性腫瘤、感染、更年期補充的賀爾蒙，甚至近年來常用的植物性賀爾蒙（大豆異黃酮）都有可能，也有研究提到疼痛是受到熬夜、壓力、食用咖啡或刺激性食品等食物所造成。值得一提的是，乳房疼痛大多會自我緩解，尤其是周期性疼痛，若是非周期性疼痛通常要找出可能的原因，也不見得需要藥物治療。

在自然療法中，有人認為維他命B6及維他命E可改善疼痛的情形。另外，一些舒緩的伸展運動也能有所幫助。就其個人的經驗來看，門診中大多會建議先觀察或是使用自然療法及運動，若是疼痛到影響生活作息，可短期服用止痛藥，如果還是沒成效，此時才會再進一步使用調節賀爾蒙的藥物。
