63歲的江媽媽和老伴因不習慣都市生活，所以兩人住在鄉下生活。江媽媽一向身體健朗，很少聽到有不舒服。但最近孩子們回鄉下探視兩老時，發現她的臉上、手上，偶爾有「黑青」，心中有些質疑，但又不敢問父母：「怎麼回事？難道深愛媽媽的老爸『家暴』嗎？」


有一天，江爸爸打電話給兒子：「我看你的阿母是中邪了！常常無緣無故昏倒，又很快的醒來。問伊怎樣了？伊攏說嘸！」兒子一聽才恍然大悟，原來媽媽的「黑青」是這麼來的；但另一方面又有些擔心是不是健康狀況出問題？於是與兄弟們商量後，力勸兩老先暫時搬到都市裡與他同住，就近照顧。


江媽媽到兒子家後，又發生了幾次昏倒事件，可是因為每次的昏倒，她都沒有不舒服的前驅症狀，而且又可以很快地自己醒來，所以她不想就醫，孩子們及江爸爸不斷拜託、央求後，她終於答應去看醫師。


神經內科醫師聽完敘述後，建議她看心臟科，並且協助她直接轉心臟內科門診。心臟內科醫師詳細問過病史，並做過初步的身體評估後，安排她做心臟超音波檢查。心臟超音波檢查發現，左心房有一個約5至6公分的黏液瘤；原來造成奶奶時而昏倒，又自己很快醒來的「兇手」就是它─心臟黏液瘤。


心臟黏液瘤是心臟良性腫瘤中最常見的，75%是發生在左心房。大小可以從直徑1到15公分不等，大多約5到6公分，發生年齡常見於30至60歲。偶發性的黏液瘤大部分染色體是正常的，只有約5%的黏液瘤和遺傳有關。女性多於男性，但如果是遺傳的黏液瘤，則男性較多。


心臟黏液瘤的患者臨床症狀表現有：體重減輕、疲倦、發燒、貧血；嚴重時，甚至會有呼吸困難、胸痛、咳血、四肢水腫、腦中風、肢體或腎臟血管栓塞、昏厥或猝死。


診斷心臟黏液瘤，心臟超音波是最好的工具；開刀切除是最好的治療方式，尤其是有血管栓塞或猝死風險高的病人。
