預測肺癌存活 台大新突破

不管在台灣還是全世界，肺癌都是癌症死因之首，殺傷力驚人。台大醫學院院長楊泮池領導的研究團隊，利用現代生物技術及生物統計演算法，找出以五種「微核醣核酸」預測肺癌病患開刀後預後的方法，對於未來癌症的個人化醫療發展，提供一個相當重要的評估平台。

台大醫學院基因體醫學研究中心助理教授俞松良表示，他們是透過內視鏡檢查或外科手術等途徑，分別取得肺癌病患的組織檢體，再進行微核醣核酸的預測，結果發現由五種微核醣核酸組成的標記，可精準預測肺癌患者開刀後的存活率及復發率。

「這真是太好了！」國家衛生研究院癌症研究所所長、中央研究院院士彭汪嘉康得知這項研究成果時，即盛讚這項預測方法是個重要突破。因為肺癌是個相當難纏的惡性腫瘤，不少患者即使接受完整治療，預後仍不理想，不僅病患自己受苦，也增加家屬的醫療負擔。

有了這套預測方法，在面對肺癌這個可怕的敵人時，多了一分「自知之明」，如果預測後續治療不錯時，不妨全力治療，一旦發覺預後很差，也許可將原本擺在最後一線的治療方式挪前，或採比較緩和的治療手段，讓病患及家屬兩相安。

楊泮池表示，微核醣核酸 (microRNA)是種新的小型核醣核酸，扮演調節基因功能的角色，在腫瘤醫學研究中，已知透過這個核醣核酸的表現，可以比訊息核醣核酸更精準進行癌症亞型分類，進而預測肺癌病患治療後的存活率。

楊泮池驕傲地說，這項以微核醣核酸表現來預測肺癌預後的技術，居於全球領先地位，研究論文已發表於本期癌症醫學領域的頂尖期刊「癌細胞」 (Cancer Cell)，而該成果也正在申請全球專利。

楊泮池強調，以往診斷方式總認為，第一期肺癌的開刀後5年存活率可達7成以上，但從他們研發的這套微核醣核酸預測方法看來，這些病患其實還可分高危險及低危險兩個族群，其中高危險族群的開刀後5年存活率明顯降低，因此必須在開刀後另加化學治療，這對病患來說，意義自是不同。

We investigated whether microRNA expression profiles can predict clinical outcome of NSCLC patients. Using real-time RT-PCR, we obtained microRNA expressions in 112 NSCLC patients, which were divided into the training and testing sets. Using Cox regression and risk-score analysis, we identified a five-microRNA signature for the prediction of treatment outcome of NSCLC in the training set. This microRNA signature was validated by the testing set and an independent cohort. Patients with high-risk scores in their microRNA signatures had poor overall and disease-free survivals compared to the low-risk-score patients. This microRNA signature is an independent predictor of the cancer relapse and survival of NSCLC patients.
