放輕鬆！凡事斤斤計較的人容易心肌梗塞

調整個性與生活態度，也能避免罹患心肌梗塞！林口長庚醫院心臟內科謝宜璋表示，臨床上發現，A型性格的人，凡事追求完美、急性子，較容易誘發心肌梗塞發作！

為患者放置心血管支架緊急打通阻塞血管部位，是目前臨床上治療急性心肌梗塞的主流和趨勢，針對血管容易再阻塞的糖尿病患者、血管彈性差又特別細小的患者，甚至於阻塞病灶較長的患者，醫師會比較建議使用塗藥支架治療以減少其再狹窄率。

謝宜璋表示，有些患者平時做事親力親為，力求完美，一刻都閒不下來，想要急著把所有工作都做完，常常給自己的壓力過大，平時忽略自身健康，對心臟也是很大的負荷。一旦急性心肌梗塞發作，如果病情輕微且急救得宜，死亡率可以降到10%以下，若發作後產生休克，死亡率將高達八成。

根據2009年最新歐洲經皮冠狀動脈治療會議(EuroPCR)上，所發表的最新研究顯示，急性心肌梗塞患者放置雷帕黴素(Sirolimus)塗藥支架，四年以後，患者再接受手術治療的比例，比裝設傳統金屬支架少了一半以上(53%)。這份研究，在歐洲、以色列、澳洲等等48間醫療院所進行，花了四年時間，追蹤715位病患所得到的結論。

謝宜璋進一步解釋，過去塗藥支架常被學者質疑支架內血栓的危險性，這份研究也針對裝設塗藥支架和傳統金屬支架引發死亡、心肌梗塞，或者支架內血栓的機率進行追蹤，結果並沒有明顯差異。四年研究期間，支架內血栓的機率雷帕黴素塗藥支架組為4.8%，傳統金屬支架組為4.4%，他說：「在統計學上沒有顯著意義，所以這份研究解除了醫界的疑慮。」

塗藥支架上所塗的藥大不相同，有支架塗太平洋紫杉醇(Paclitaxel)抗癌藥物，另一種就是雷帕黴素塗藥支架。謝宜璋說，裝設金屬支架病患血管至少有20%會再狹窄，而目前最早上市的塗藥支架，已有近十年年的使用經驗，可以將血管再狹窄率降到3%-5%以下，並且取得歐盟CE認證標章，代表通過國家衛生單位的實證與考驗。

要防止支架內血栓產生，患者都需要好好配合醫囑，定期服用保栓通或者阿斯匹靈等抗血小板凝集藥物，才能降低支架內血栓的風險，也許有人覺得服藥很麻煩，但裝設支架打通阻塞血管，不僅可以救命，也可以維持患者良好生活品質。
