聽聲觀相 慢性咳嗽有解

長期咳不停，問題出在哪？醫師說，咳嗽超過3個星期以上的慢性咳嗽，最常見原因為鼻涕倒流、氣喘及胃食道逆流。可依照夜咳發作時間、說話、唱歌、聲音及面相幾點特徵，簡易判斷原因。不過，最後仍應該找醫師做診斷，接受治療。

長庚紀念醫院胸腔內科系主任郭漢彬表示，如依照夜咳發作時間分辨，若是一睡下去就開始咳，且是短促、頻繁的乾咳，原因可能出在「鼻涕倒流」；至於氣溫低或在睡後3、4小時開始咳，夜咳加重，則問題恐在「氣喘」；在睡著放鬆的狀態下，不定時咳嗽，則可能有微量胃酸吸入呼吸道引起刺激，及「胃酸逆流」至食道中段，刺激迷走神經，引起相關症狀。

若依照「聲音」分辨，鼻子問題所造成的，說話或唱歌會有痰音；若是胃食道逆流引起者，說話尾音顫抖、唱歌高音唱不上去，或較以往高音降低一至兩個key，說話聲音沙啞，且容易失聲。

依照「面相」觀察：鼻子問題所造成的長期咳嗽，患者平時習慣張口呼吸、口乾舌燥、雙眼內側容易有黑眼圈；至於胃食道逆流所造成，容易門齒腐蝕，呈現鋸齒狀，主要是因為胃酸逆流至口腔，長期侵蝕所造成，舌頭容易潰瘍、口臭等。

長庚紀念醫院胸腔內科呼吸道疾病科主任王圳華表示，真正難治療的慢性咳嗽，是胃食道逆流所引起，高達4成患者沒有症狀，加上一般人對胃食道逆流所引起的呼吸道疾病認識不深，也常會誤以為是一般感冒，導致無法對症下藥。

王圳華說，8成以上患者可有效改善咳嗽及胃酸逆流等症狀，僅兩成無效，主要是與生活作息及個性有關，因此患者除控制用餐時間和食物外，最重要的是，減低生活壓力，調整情緒，才能回應身體所發出的警訊，找回健康的生活。
