三大亂象！台灣麻醉致死率高

一個台灣醫界都知道這個公開的祕密──台灣的麻醉品質崩壞中！

根據台北榮總副院長何善台等人發表在《台灣麻醉醫學會期刊》的文章，2002到2008年間，台灣的麻醉相關致死率平均達十萬分之十二，約是日本的12倍、英國的21倍，更是美國的12到24倍。

台灣不少麻醉亂象，猶如上演一齣齣黑色鬧劇，背後都是嚴重的結構性問題。

麻醉亂象１：超量工作 徒增致死率

「麻醉的困境之一就是人力不夠，品質當然下滑，」台灣麻醉醫學會理事長、童綜合醫院副院長廖文進急切地說。

攤開麻醉科執業醫師人數，有900多人。但廖文進指出，目前有近200人已改行，真正執業的只700多人，加上各醫院紛紛推出需麻醉的自費項目，又稀釋掉能投入開刀房的麻醉醫師人力。

依據國防醫學院生命科學研究所博士王如娥等人，去年發表一篇針對麻醉科醫師做的調查，發現麻醉科醫師一天同時平均得兼顧4台刀，但麻醉科醫師自認3台才合理。

廖文進私下也觀察到，有三分之一的醫院麻醉科醫師，其實一次兼顧超過5台刀，甚至10台刀的，也大有人在。

過量的工作，使得台灣麻醉相關重大併發症，超過五成肇因於人為因素，可事先預防。

一位十幾年資歷的心臟外科醫師坦言，因為麻醉醫師要顧的刀太多，他的手術每個月平均都會出現一、兩次，病人的呼吸有問題，甚至要CPR急救的狀況。

「政府要正視這件事！」接受採訪時，他一直嘆氣。

麻醉亂象２：兼差賺外快 集團跑單幫

醫美診所、健檢中心、婦產科診所等，都需要麻醉人力，也讓加入這個市場的人力愈來愈複雜。

「麻醉變得很黑了，」一位診所醫師搖頭感嘆。

目前，已陸續有各大醫院專任的麻醉科醫師，利用值班隔天休假時，背著醫院與衛生局，在沒有報備支援下，違規偷偷出來兼差「跑江湖」。

一位有幾年跑江湖經驗的麻醉醫師坦言，「跟診所拿現金，有的現場給、有的月結，我們都不走戶頭，免得被國稅局盯上。」

也有麻醉醫師帶著幾個麻醉護理師，或麻醉護理師自成集團，包下幾家診所、醫院的麻醉業務。「這是違法，但很難抓，」另位麻醉醫師感慨，目前太多以合法掩護非法的狀態。

事實上，健保給付偏低，也讓麻醉醫師一次只顧一台刀的要求，變得不切實際。

以給付最高的全身麻醉來說，一台平均兩小時的全身麻醉手術，健保給付約3912點，乘上健保總額預算的浮動點值，打個8折、9折，醫院得到3,100到3,500元左右的給付金額。

麻醉亂象３：時薪700 淪為「低價勞力」

醫院再把這3,000出頭的金額，提撥三至五成左右，付給麻醉醫師。

做兩小時的全身麻醉，如從頭到尾一次只顧一台刀，換算下來時薪不過700、800多，可能還比不上國立醫學院學生當家教領的時薪。

更何況麻醉是高風險的行為。

「麻醉醫生是把一個活人（在手術中）變成死人，（手術結束）再變成活人，我們踩在陰陽線上，」台南市立安南醫院麻醉部主任蘇翔，以此說明麻醉醫師的價值與重要。

但現實逼使麻醉科醫師必須同時兼顧好幾台刀，把照顧病人的工作大量移轉給麻醉護理師，才能維持一份不錯的薪水。

想要提升麻醉品質？情況恐怕是更雪上加霜。

不論台灣的麻醉死亡率是十萬分之幾，對任何動手術的病人來說，就只有一（安全度過）與○（發生意外）的差異。沒人想要承擔最不好的結果。台灣的麻醉品質，必須急起直追！
