自行車手/媽媽手/電腦手，都是同一手嗎？

最近迷上騎腳踏車。為了配合節能減碳並減肥健身，我固定每週一兩次從家裏騎二十幾公里到醫院上班；開始實施之後才發現不同的階段需要克服不同的狀況：



騎車練功第一關：屁股痛

剛開始的時候，屁股疼痛是最大的問題，終於瞭解為什麼自行車雜誌上會說有些比較遠的距離叫做「鐵屁股行程」。不過這些問題隨著里程數的累積，似乎已經解決了，也就是網路上人家說的，已經練成了鐵屁股。現在就算騎到腳快斷掉，屁股也不會太痛了，但卻又發現了另一個問題。　



騎車練功第二關：手麻了

我發現只要騎自行車到一定的距離，手就會開始發麻，這症狀完全符合我們骨科醫師在臨床上看到的「腕隧道症候群」。但是多年的行醫經驗裏，卻從沒有人是因為騎自行車造成的腕隧道症候群前來開刀；也就是說，這並不是自行車族的通病。

那，難道就是因為我老了嗎？“小醫”已經變成“老醫”了嗎？好不容易我終於找到了一個不容易產生運動傷害、也不用找伴的運動，我竟然也不能做！？

搜尋了一下網路上國內外關於自行車及其所造成手部神經問題的資料才發現，原來在國外早已有了「Cyclist’s Hands（自行車手）」這個名詞，而且還分成因平把（休閒或登山車）造成的「正中神經障礙」或彎把（公路車）所造成的「尺神經障礙」。


在這裡，我就把這兩種情形稍事介紹一下吧！

1. 正中神經障礙：

平把車單車，因為會過度讓體重壓在手腕及手掌上，再者腕部也會因為過度背屈造成腕隧道壓力上升，導致正中神經掌管的位置：包括大姆指及食指中指無名指掌側就會酸痛，甚至於麻木無力，但是，只要改變位置及姿勢就可能改善。

一般嚴重的腕隧道症候群甚至連晚上睡覺時都會痛醒。病人典型的抱怨是：騎機車握把手在摧油前進的時候，就會陣陣麻木。最後可能需要內視鏡手術解離腕隧道以降低正中神經壓力。


2. 尺神經障礙：

彎把公路車，腕部必需向大姆指側彎曲，因此當長時間握緊彎把最前端時，常會造成小指側的尺神經過度伸展壓迫在彎把上，以致造成尺神經掌管的位置，也就是小指側包括手指及手掌的麻痺。同樣的，一旦改變位置及姿勢就會有所改善。但是，在臨床上嚴重的尺神經障礙患者除了麻木外，還可能會造成手掌肌肉的萎縮變形；屆時保守治療無效的話，也可能要手術因應。　



7大方案解決“麻麻的”自行車手

所以，想提醒和小醫一樣喜歡馳風的自行車手們，如果您已經開始感覺到有這樣麻木的問題時，主要的解決辦法有：

確定您的腳踏車適合您的身材：不適合的腳踏車尺寸，可能會造成體重過度壓在手把上，而產生不良的影響。

每十到十五分鐘就要改變手在握把上的位置：改變姿勢可以改善血液循環及減少神經壓力。
穿戴自行車手套：手套上的緩衝墊可以減低壓力。
使用人體工學把手或將平把車裝上牛角：以增加把手可握的位置。
使用好的手把膠帶或使用碳纖維手把：可降低路面傳來的震動。
改善姿勢：手肘微彎約十度，不要將手重壓在腳踏車的把手上。
平常做一些手腕的伸展及肌肉訓練：只要上網找腕隧道症候群的自我復健運動就可以找到了。
看起來，騎自行車騎到手要開刀的機會似乎不大，只要利用一些方法，手麻這個問題似乎可以迎刃而解。　



騎車練功第三關：尚待體驗中…

但還有一個問題：據說騎腳踏車會引起內陰神經（PUDENTAL NERVE）壓迫造成某種功能問題。不過，我看還是留待下次再談吧！等我有親身經歷時再和各位分享，可能更真切！

【註】：
電腦手：因長時間使用滑鼠，過度背屈手腕造成的腕隧道症候群，成因類似於平把車造成的自行車手。
媽媽手：手腕因為長時間抱小孩過度往小指側彎曲，造成大拇指伸指肌腱發炎疼痛，不是因為神經壓迫的問題。
