活血與行氣併用　中醫傷科治療看不見的「外傷」

衛生署新竹醫院中醫科黃雪雅醫師表示，一位年輕男性患者因為近兩周全身痠痛無法入眠而至其門診就醫，問診得知，患者在一個月前因騎機車被追撞跌倒，但當時就診西醫時並無發現骨折或其他外傷，因此僅口服止痛劑，未再進行其他治療。但是患者卻在約半個月後出現全身痠痛的症狀，疼痛感在晚上更加明顯，嚴重影響睡眠品質，而且還出現胸悶、骨頭及關節有酸酸、重重的感覺，這些都是身體外傷的後遺症。經用藥且輔以傷科推拿後，痛感消失，身體也變輕快。


黃雪雅醫師表示，人體在遭受外力撞擊或跌倒後，內部經絡氣血會出現短暫的紊亂現象，輕微者會自我調整回復到正常狀態，嚴重外傷則需要藉助傷科推拿手法以及傷科用藥來治療，以去除外傷導致的內部瘀血或氣鬱現象，使得氣血運行順暢，經絡循行回到常軌，身體出現的痠痛自然得以緩解。


有些慢性酸痛症狀的病因是來自於多年陳舊性的外傷所導致，因此在用藥治療上「活血」與「行氣」必須併用，如桃仁、紅花、川七、枳殼、柴胡、牛膝、木瓜等都是臨床上常用藥物。像川七具有活血及止血的雙向治療作用，無論在傷科或心血管疾病患者都是相當重要的藥物之一。


黃雪雅醫師指出，瘀血所造成的疼痛多在夜晚發生，像這位患者在用藥上也是以活血化瘀、行氣止痛為主，經用藥二週後，病患自訴其痛感消失，身體也變輕快，這是由於治療後全身氣血順暢所致。除傷科用藥外，傷科推拿手法的運用也相當重要，對於兩週以內的急性病症及經年累月的陳舊病症會採用不同的推拿手法，以外治手法加上內服藥相輔，對大多的酸痛病症都有一定的治療效果。


除此之外，中藥外洗與薰蒸治療也是中醫傷科的一大特色，可藉由活血藥及補益肝腎的藥物，經皮膚吸收直接對外部淺層經絡做治療，對於急性期患者療效顯著。因此，黃雪雅醫師表示，各類急慢性痛症除了止痛藥或肌肉鬆弛劑外，其實中醫傷科治療能更完整的對人體經絡筋骨進行調節及修護，避免出現外傷後遺症，讓骨骼、肌肉都能健健康康的陪伴一生。

我認為那不叫做外傷，而叫做over use, fatigue, malasia等
在衝撞的時候，比如說right hand landing without colle's fracutre
雖然未骨折，但是衝撞力有可能使得biceps tendon受傷
而且在跌坐的過程之中也會有全身的衝撞
都可能造成拉傷
一般的中醫師不查的話，就容易歸類到「氣未順」這一種說不上什麼是什麼的病因裡頭來
沒有傷是無緣無故的
細查原因應該就能探究出來
顯而易見的如衝擊點的挫傷扭傷，這些是容易診斷
但是其他的就容易忽略
在治療上，久傷，包括有診斷出來與未診斷出來的，時間一久就成了久傷，都是以活血化瘀藥為主
理氣藥為輔
經驗上效果都是不錯
但若已經造成結構變異，比如 bankart lesion，那麼最好的治療還是關節鏡
等做完關節鏡二三個月之後，疼痛只剩頑固的、一點點的固定疼痛，但ROM都沒問題時，但復健療程不再進步時
這時再用活血化瘀藥+理氣藥
再加上原已無進步的復健療程或是針灸、推拿
能得到的效果當比不使用中醫手法介入來得好
總歸一句：中醫介入時機巧，造就名醫真不少
