愛滋病年輕化 百億醫療費全民買單

【台灣醒報記者游清淵台北報導】12月1日是世界愛滋日，截至今年10月，台灣愛滋病通報確診的本國籍人數已高達33018人。衛福部明確表示，自2011年開始的5年防治計劃中將投入207億元經費，主要是因為性行為感染人數成長迅速，尤其是男男間的性行為因生理結構因素，較陰道性行為感染率高。透過昂貴的雞尾酒療法「救命」，公費在2012年底已欠健保署28.6億元。

感染數超越去年
台灣已有33018愛滋累積案例，依據疾管署統計，2016年截至10月獲通報案例已有1991例，根據過往累積案例中男性有31083人、佔94.14％，女性則僅有1935人、佔5.86％。疾管署曾在7月表示，推估今年愛滋新增感染人數將超過2015年的2328人，感染愛滋者有86.9％是因不安全性行為所造成。

衛福部公開的「愛滋防治第5期5年計畫」中載明，男男間性行為感染愛滋一直是愛滋防治重要的議題，且感染人數每年持續上升，2007年占總通報數40％、2008年占58％、2009年占67％、2010年占71％。

感染年輕化堪憂
愛滋病感染有年輕化趨勢，2014年1月至6月通報的愛滋病毒感染者中，近三成為15至24歲，其中近九成都是透過不安全性行為途徑感染，顯見年輕族群之愛滋病防治工作有待加強。愛滋病已成為青少年十大死因，九成的愛滋感染者皆源自於男男性行為，起因於不使用保險套，一方面為表現忠誠度，一方面使用保險套會造成性行為不適。而且其性行為方式也容易造成保險套破損而感染的機會增大。

「全球至少有5至10％的愛滋感染是發生在男男間的高風險性行為。」衛福部的報告中指出，因生理結構的不同，致使男男間性行為之肛門性交行為，較陰道性交行為感染愛滋病毒的機率高，在美國有54％的愛滋感染者為男男間性行為，而在台灣2010年的累計感染率有20801人，感染途徑分布為男男間性行為有8817例、占44％為最高、其他依序為注射藥癮者6448例（32％）、異性性行為4403例（22％）、血友病患53例、母子垂直感染29例、接受輸血21例、資料不詳286例。

對愛滋病友關懷
台灣愛滋病學會理事長林錫勳指出，國外研究追蹤1166對其中1人是愛滋感染者的情侶長達1.3年，發現情侶間只要「按時」服藥控制良好，期間發生5.8萬次的無套性行為都沒有造成另一半感染，愛滋病毒的傳染途徑是高風險性行為、輸入愛滋感染者血液、共用針頭等方式，盼社會零歧視。

雖然按時服用雞尾酒療法藥物能有效控制愛滋病友傳播感染，但由於醫藥費十分昂貴，在衛服部的報告中指出，2009年的醫療費用約19億元，且近5年的費用每年均成長16.3％，造成政府財政龐大負擔，以全民買單的「公務費用」為項目做醫療支出，在2010年已積欠健保局13.4億元，至2012年已有28.6億元的赤字。
