外科手術前刮毛

本來我們不在病房刮,送開刀房後,就在手術前才刮...當時是想到...事先刮的話,如果傷到皮膚,讓細菌入侵傷口,對手術不利...
最近來了一個外科醫師,堅持要在病房刮好毛再送....

我不是外科醫師,但覺得毛毛的,請問有evidence based medicine或guideline提到外科刮毛的事嗎 ?

通常是跟手術部位有關吧？
術後會有傷口的地方那附近就會先把毛刮掉
不然術後有毛,對換藥和傷口照顧都不好

至於誰來刮.....有的醫院是intern,
有的是R....不一定吧？

我聽說過有一招
是如果病人可以自理的話
叫他自己刮？？？

後來當R ,VS時都是麻醉完才刮毛
很少聽說前晚刮了

我的重點不是講刮毛史....我是說....學術上,好像拔毛比刮毛好...真要刮的話,不要離手術時間太久,像上述的前一晚刮,應該是錯的喔 !

記得有看過，因為刮完毛後毛囊內的細菌會滋生
隔越久細菌會越多

刮完愈久，infection的rate愈高，
建議的作法是下刀前30分鐘內shaving，
所以我們的routine是在OR麻醉完後刮~

而且建議使用電動除毛刀代替剃刀，避免傷到表皮，容易引起感染

我之前剛好有看過一篇NICE guide關於減少手術感染的做法還蠻有趣的。節錄在我的blog:http://tayuwu.blogspot.tw/2011/07/nice-guideline.html

剃毛不能減少傷口感染，如果用刮鬍刀、剃刀剃毛，反而可能有微小的損傷而"增加感染"，如果手術部位需要剃毛，建議使用脫毛膏或是電動剃刀(拋棄式刀頭)，用電動剃刀的CP值最高。
