（中央社記者陳麗婷台北5 日電）健保局實施DRGs同病同酬包裹式給付已3 個月，部份醫院表示，碰到複雜度較高的病患，像是心肌梗塞、換心導管患者，住院天數一定超過，多付的成本還是得醫院自己吞下。

健保局今年初實施「住院診斷關聯群」DRGs同病同酬包裹式支付制度，實施3 個月後，有些醫院抱怨不敷成本。

新光醫院心臟內科主治醫師趙書平今天受訪表示，以裝心導管為例，健保只給付一定的定額，如果碰到 1個病情比較危急，像是心肌梗塞或必須使用主動脈氣球幫浦的病患，住院時間一定會拉長，治療成本也一定會超過健保局支付的定額。

但趙書平說，這是沒辦法的事，還是要以治療患者為重。他舉例，1 週可能會收治狀況較簡單或複雜的病患，只能以簡單的個案來補複雜個案治療的成本。

石岡診所院長趙凱受訪表示，若以盲腸炎來說，盲腸破，若再加上患者本身有糖尿病，治療非常麻煩，風險也較高，患者可能一住就是1 個月。有些小醫院將患者治療1週後，可能就轉往公立、醫學中心繼續治療。

趙凱說，病患被轉院權益也可能因此受損，健保局將在3 個月後進行檢討，屆時才會真實反映到底對醫院有多大的影響。
