憂鬱症治療須半年 勿擅自減藥

台安婦女身心醫學科醫師張育彰表示，受到產後荷爾蒙變化、嬰兒照顧及家人相處等問題的影響，根據統計，約有一到二成產婦會罹患產後憂鬱症，甚至衍生產後精神病，此時便須高度注意預防「殺嬰」悲劇發生。


醫師指出，極少數極端個案會從產後憂鬱症惡化為產後精神病，此時患者思考、知覺功能都會發生嚴重變異，甚至出現幻想、幻覺等症狀。


例如，懷疑孩子不是自己親生的或遭人掉包，進而衍生傷害，甚至與寶寶同歸於盡的念頭與行動。


張育彰說，廣泛的產後憂鬱症可以涵蓋輕微的產後情緒低潮、莫名哭泣，症狀多在產婦生產後幾天內出現，但一般只要經過短時間自我調適，即可不藥而癒。


但真正的產後憂鬱症不一定會在產後何時發生，且多會持續一到二周，甚至數月之久。常見症狀包括說話明顯變少，對周遭任何人事物都缺乏興趣，甚至會有輕生的念頭，或認為自己沒有資格當母親。


張育彰強調，研究顯示，原本就有憂鬱症病史的婦女因產後各種新增壓力，憂鬱症復發機率相當高，但嚴重度是否有加成效果，則因人而異。


一般來說，憂鬱症治療的基本療程約須六個月，期間未經醫師同意，絕對不能擅自減藥或停藥。


否則，患者不但隨時有可能發病，發作頻率也會愈來愈頻繁，症狀也會一次比一次更嚴重。
