鄉醫院推「在宅往生」 盼增醫療資源

台灣逐漸邁入高齡化社會，但偏鄉和城市醫療資源不均，居家、社區的長期照都缺乏醫療資源，是一大問題。民進黨立委陳節如上午舉辦「台灣社區醫療照顧分享座談會」，邀請台大醫院金山分院、七堵社區醫療群分享社區醫療經驗。

台大醫院金山分院長黃勝堅表示，長者多半希望在居住環境往生，而「在宅往生」也非口號，金山分院目前已發展「接力賽照顧模式」，在長者臨終前4天至13天，團隊將赴家中密集照護，即便在瀕死邊緣，病人和家屬都會安心。

七堵社區醫療群執行長紀乃正指出，醫療群5年前創立後，配合志工服務，每月都至基隆的瑪陵、友蚋等偏遠山區巡診，每次服務25人。不少參與醫師都認為，走出白色巨塔在病患家中與他們互動，能夠更拉近彼此。

不過陳節如說，很多地方一個月僅有一名醫師巡迴診療，她說，失能者或長照者並不如一般病患，不能夠自行告訴醫生「我哪裡痛」，也無法自行求診，需要有專業的社區服務人員，視長者狀況安排「醫療套餐」。

陳節如指出，以台大醫院金山分院為例，雖已從事在宅醫療，但仍缺乏慢性病、牙醫、營養師等職位，盼未來能繼續擴編，增加社區長照的醫療品質。(郭建伸／台北報導)

醫師要強調按照法律規定及家屬意願，要拿到「書面」的同意或不同意書才能更改，
請讓強勢的家屬，自行去喬弱勢的家屬簽更改同意書，
