理性支持健保永續

衛生福利部掛牌成立，首件新聞卻是健保的部分負擔擬調漲，已經有人預言將面臨反彈，還有媒體大肆渲染「窮人又要被剝層皮」。景氣不好，要老百姓從荷包裡多掏錢出來，當然不可能討好。不過，健保不能倒，是今天這個紛擾社會的難得共識。如何讓健保永續發展，格外需要大家用理性、不要只顧著小我的態度來探討。

健保的形象「俗擱大碗」，卻是用魔鬼代價所換來的，包括壓低醫療專業人員的給付。其結果，醫學院校出現「四大皆空」，護士掛著點滴勉強上班的畫面屢見不鮮，讓醫護變成「血汗行業」，這於台灣當前和未來的醫療品質，是多麼可怕的警訊！健保資源的不合理分配，非改不可。

該改的事千百種，最難改的可能和民眾的就醫習性和心態有關，例如台大醫院近年提倡減少病人生命末期的「無效醫療」。另一事則相關台灣人年平均就醫次數，比起先進國家，實在超出太多到不合理的程度。而向來就存在著部分國人喜歡「逛醫院」的說法，對照健保「真便宜」的印象，這種醫療浪費實應遏止。

這次提出的調漲部分負擔方案，說不上「激進」，對幼童、低收入戶、重大傷病者都排除在外，對65歲以上老人也要達到一年看診超過61次以上，才會增加自付部分。這一方面於健保收入是「開源」，如果還能因此引導改變民眾不必要的就醫、重複就醫、及太輕易跑大醫院掛急診的習慣，減少醫療浪費，也會有「節流」的作用。收入不高者當然更叫痛，但健保早已遠離「保險」精神，用近乎「健保稅」的手段，讓低所得者低繳健保費，本已具福利性質。這樣一小步改革，應值得推廣全民承擔吧？

老實說，現在政府欠威信，任何政策影響到民眾荷包的，一推出就被罵得狗血淋頭。年金改革不是如此？油電雙漲不是如此？弄得政府畏首畏尾，什麼都不敢做。但「今天不做，明天後悔」的事太多了，若照現況延宕下去，十年、二十年後台灣將無可用的醫生、護士，再來喊救命，也緩不濟急吧！我們呼籲社會理性支持健保的永續改革。

真正理性是要誠實面對自己政府財源力有未逮.
誠實告訴人民政府承諾人民可以天天吃牛排是理想卻不是事實.
政府所要做的不是尋求財源而是如何健保瘦身,也就是忘掉包山包海的幻想.這才是理性改革.
