明年一月一日實施的二代健保費率確定採取四．九一％，如果營運順利，一○五年之前都不調費率。

衛生署提出的二代健保費率建議案包括四．九一％、五％、五．一七％等三案，在油電雙漲、薪水不漲的惡劣環境下，行政院擔心採取高費率會加重民眾負擔，引發民怨，決定採取最低費率。行政院長陳冲昨指出，二代健保案是非常辛苦、高難度的改革，「注定沒有掌聲，但非做不可」，希望各部會共同努力，預料最快今天政院會核定宣布。

6項補充保費 明年上路

二代健保明年一月一日上路，保費除了現行由薪資計算的「一般保費」之外，將再加上「補充保費」，計費項目包括高於四個月的獎金、兼職所得、租金收入、執行業務收入、股利所得及利息等六項，除了獎金之外，其餘五項補充保費下限為五千元、上限為一千萬。

衛生署估計，採行四．九一％較現行費率五．一七％低，將有八成以上民眾的健保費會下降。其實不然，由於政府將一併取消現行對收入較低民眾的「健保費率調漲差額專案補貼」，預計將有六成以上民眾的實質保費支出，不降反漲；高所得者多會有股利等其他收入，亦得多付補充保費，形成「全民皆漲」。

督保盟：4.91％是破產費率

除了人民負擔加重外，督保盟發言人滕西華抨擊，四．九一％根本是破產費率，勢必導致健保收入不足，醫療支出必須縮水因應，醫院為節省點值，只能紛紛關診因應，最終還是影響民眾就醫權益。

滕西華指出，根據健保局的二代健保費率精算，政府負擔比率應達三十七．五％，但政府卻只願付最低負擔比率三十六％，二代健保一開辦，收入硬生生就減少一百四十八億。行政院選擇了最低費率四．九一％，衛生署不敢面對健保財務撐不起的事實，硬是吃了下來，官員要為自己的政治決定負責。

根據健保局完成一○二年二代健保費率精算，費率四．九一％，可收得四千五百六十三億元，費率由現行五．一七％降至四．九一％，一年將少收入二百零八億元，但補充保費可收回二百億元左右，再加上政府增加挹注部分，足可支應至民國一○五年。

取消補貼 6成保費增加

取消現行「健保費率調漲差額專案補貼」後，人民要多付多少錢呢？目前國內有一千一百五十三萬名民眾，月薪在四萬零一百元以下，每人每月保費將增加十九元到四十四元不等，一家四口就要增加七十六元至一百七十六元，第六類失業的民眾每人每月要多支出九十元。

至於月薪在四萬零一百元以上至五萬零六百元的民眾，每人每月估計雖可少繳十六元到一百四十二元不等，但高收入者大多投資股票、銀行有存款，在加計「補充保費」後，整體保費也會比現在高，等同「全民皆漲」。
