遇到挫折，要放下面子


另一個例子則是一位女住院醫師，已到院四個月，有一回為病人進行腹部超音波檢查，結果做了一個半小時，卻都看不出所以然。病人因為得把衣服拉到胸口才能做腹部超音波，不禁說了句：「醫師，我很冷！」沒想到這句話竟讓女醫師放下手上的工作，委屈地跑到護理站哭了。


護理人員認為事態嚴重，便向我反映。隔天我也試圖了解一下究竟發生什麼事，心想也許是遇到診斷困難的案例，第一年住院醫師腹部超音波不太會看，是很有可能的，若真的不太會看，也可以找別人幫忙看；結果腹部超音波看不出所以然的原因，竟然是「打不開那部超音波機器」，我又是一愣，問：「你真的花一個半小時，為了打開機器？」她低頭默默無言。


其實病人只是說了句「很冷」，並沒有責備的意思，但她聽到這句話就受不了了，產生了挫折感。明明知道病人沒有講錯，也知道自己搞不定那台機器，卻不願意拉下臉尋求協助，這個反應令人錯愕。進一步了解住院醫師的背景，原來她是家裡第一個當醫師的人，從小品學兼優，課業表現名列前茅，就讀醫學系時，成績是系裡的前百分之四十，是可以選擇六年畢業的優等生，而且以第三名的成績畢業。


我告訴她，不熟悉機器的操作，不知道開關在哪裡，有很多方法可以臨機應變。考慮到她可能因為成績向來很好，不習慣求教於他人，我試探性地問她，是否曾考慮要請教同學？果然她的反應是「完全沒想到」。在她的人格特質裡，凡事都是靠自己解決。但人非萬能，並非所有事都靠自己便能成功，若能力有限，能找到一個幫忙解決的人或是方法，也是一種「靠自己」的做法，學習總會有學習曲線，不是什麼事情都是 Do It Yourself（DIY），就能成功。


自尊，是許多醫師的大問題。她無法在病人面前說「這個我看不懂」，「這個我不會」。中國人向來好面子，各行各業都有這種「愛面族」，而這群成績好的醫師們又是其中的「佼佼者」。我幫她想到的第二個方法，是教她以後若遇到同樣的情形，又要顧及面子，在病人面前能下得了台，不如就罵機器太爛了，反正機器不會答腔，邊罵邊趕快把機器推到護理站，問一下其他護理人員如何操作機器，開關在哪等等，搞定了再推回去，就把問題解決了。


既是家族中第一位醫師，我說：「這一路學醫過程的辛酸，可能父母親或身邊朋友都不太清楚吧？」她聽了，眼淚立刻流個不停。後來女醫師也坦承，考試對她來說，太簡單了，但臨床看診時面對病人，自己總是摸不到頭緒，抓不到重點。從幾個月的住院醫師經驗，她發現，臨床最大的困難，是自己不太會跟病人溝通、解釋病情，面對病人時壓力很大。


我站在內科立場，當然希望優秀的後輩可以留在科裡，但如果她無法適應臨床，我建議她再觀察自己一陣子，倘若面對病人還是覺得困難，倒是可以選擇一個不需要直接接觸病人的科。隔年，我輾轉得知，她轉到了病理科。臨床是猜測的醫學，可能對病名、病因都不清楚，必須從治療來觀察病人反應，決定治療的方向是否正確；而病理科是臨床的最終診斷（final diagnosis），每一科得到的成就感不太一樣。她能發現困難，找到出路，是好消息，從一次的失敗確認從醫方向，給自己一個機會，是值得慶幸的事。
