糖尿病患泡湯 小傷口險截肢

天氣冷颼颼，不少人喜愛泡湯驅寒，不過，一名50歲的糖尿病患卻因為泡腳泡過頭，不僅把雙腳燙掉一層皮，還因為傷口久久不癒合，險些失去雙腿。


醫師指出，慢性傷口一旦處理不好，對患者、照護的家人與健保財務都是很大負擔，糖尿病等慢性病患應做好足部保護，有傷口時，應儘速尋求專業團隊處理。


泡20分鐘 皮膚竟壞死

據了解，該名患者當時是與好友去做足浴泡湯，泡腳泡了10-20分鐘，渾然不覺有異狀，一直到按摩人員做後續療程時，才發現他的足踝以下皮膚已經發紅、起水泡。醫師檢查發現，患者足踝以下的皮膚已整層壞死。


雖然緊急進行清創手術，但術後傷口癒合仍然不良，敗血症、急性腎衰竭接踵而來。經過醫護人員長達半年的努力，才終於保住雙腳康復出院。


治療半年 終保住雙腳

長庚醫院整形外科主治醫師馮中和指出，人口老化使得慢性傷口的發生率跟慢性病一樣逐年提升。根據統計，接受小腿截肢的糖尿病患中，有90%是因為慢性傷口引起。


除了糖尿病患的足部傷害容易形成慢性傷口外，長期臥床患者的褥瘡、靜脈曲張患者的足部潰瘍等，也都可能因為患者的循環不好，發生的部位剛好位於皮膚相對薄的地方，或是傷口比較深等因素，而形成慢性傷口。


潰瘍傷口 原是皮膚癌

像是今年85歲的游老先生，二次大戰時子彈打在他的足踝上，傷處形成皮貼骨的狀態，以致傷口總是好一 陣子、壞一陣子，足足困擾了他40年。


而79歲陳老先生的腳上「傷口」，最後還被檢查出是皮膚癌。陳老先生的太太說，先生的小腿背上傷口一直在「流湯」，天天用消炎藥膏擦，持續擦了好幾個月，傷口才癒合。


本來以為沒事了，沒想到傷口部位卻漸漸隆起兩個像是樹莓一樣紅紅的腫包，由於先生以前得過大腸癌，她擔心可能是不好的東西，催促著先生就醫。沒想到醫師切了組織化驗，發現竟然真的是癌症。


醫師指出，陳老先生一開始的潰瘍傷口可能就是癌組織，只是老先生誤以為是撞傷。所以說，面對長時間不癒合的傷口千萬不能輕忽。


有鑒於此，長庚醫院成立慢性傷口門診中心加強醫療服務，希望喚起社會大眾重視慢性傷口的問題。


長庚整形外科教授楊瑞永指出，慢性傷口門診中心同時具有治療室的作用，患者經由專業醫師診斷後，包括清創等治療都可以在診間進行，可以減少患者奔波久候。
