二代健保收支連動 恐仍入不敷出

為導正過去財務平衡機制運作不良而實施，造成健保連年虧損的現象而實施的二代健保，最主要的變革就是促成負責法規諮詢、收入費率的健保監理委員會，以及處理預算支出的費用協定委員會「兩會合一」，建立「收支連動」的機制。

現階段，收支連動成效尚無法評估，但各界普遍擔憂，未來全民健保仍然可能再度面臨入不敷出的窘境。

一代健保時期，健保監理會與費用協定會獨立運作，卻發生兩造溝通窗口未盡暢通，造成健保收支不對等的情形；自102年兩會合併為「全民健康保險會」後，即可量入為出，直接由內部協商收入與給付醫療體系的問題。

醫改會研發組研究員沈珮涵表示，監理會與費協會每年7至9月時會展開健保總額協商，就當年度健保支出的情況，討論明年度西醫、中醫、牙醫、醫院、洗腎等5大部門的分配比例及總額成長率，也就是今年的健保收支，是去年兩會分立時期決議的結果，因此必須等到明年，才能真正評估收支連動機制的成效。

政治大學公共行政學系主任陳敦源指出，伴隨二代健保加收的補充保費，為健保挹注了300億元的準備金，甚至遠超過當初預估的200億元，顯示長久以來的虧損情形並沒有發生，因此目前收支連動的機制還不需要發揮功能，但人口老化問題日益嚴重，「老年人所使用的健保資源是中壯年人的6倍，」陳敦源說，「未來健保總額勢必繼續提高。」

「收支連動運作的基本概念，就是透過縮減給付面或提高費率，維持收支平衡。」民間監督健保聯盟發言人滕西華表示，新藥及新療法日新月異，健保給付項目幾乎只增不減，縮減支出難以達成，但馬政府傾向不再調漲健保費率，因此只能增加政府負擔比例，以因應未來繼續提高的總額。

醫勞盟副秘書長林秉鴻則指出，今年健保創造高額「盈餘」，未來即便啟用收支連動，也難以再造榮景，「如果不是所有醫護人員共體時艱，忍受極低的總額成長率，健保以公家給付的標準，根本無法提供私人等級的服務。」

「健保可以採取社會保險與特別稅『兩制並行』的模式運作。」林秉鴻解釋，健保加入特別稅制，既能增加收入，也能使「富人」承擔更多保費以符合公平原則；衛福部社會保險司長曲同光認為，政府目前負擔健保總額的36%就是來自稅收，「我不建議再開闢另外的稅制來支持健保。」

台灣醫師公會全聯會理事長、立委蘇清泉表示，明年度的健保收支情形，是對收支連動的大考驗，各界實在不應該把一代健保時期提高費率，以及二代健保時期收取補充保費，所得到的300多億元視為「盈餘」；而且以健保署每個月需要300至400億元的正常支出，以及健保成立之初所規劃的3個月準備金標準，「300億元其實還不夠，頂多是避免債留子孫而已。」

針對二代健保扭轉虧損劣勢，基層醫界反彈聲浪卻日益高漲的問題，蘇清泉解釋，部分健保會付費者代表的觀念還停留在費協會時代，完全以壓低支出為考量，「雖然說壓低各部門的成長率，可以將健保總額限制在一定的範圍。」蘇清泉說，「但過度壓縮的結果，就是連維持正常醫療服務的空間都沒有了。」
