牙痛、感冒掛「急」診 醫院恐說NO！

你曾經因為感冒拉肚子看醫生，因為想快速方便，或是門診掛不到號，情願多付錢看急診嗎？健保局為了舒緩急診壅塞情況，將擬定只要是第五級、非緊急病患，掛急診，比照一般門診；也就是說，明明是門診時間，但小病痛卻掛急診，醫院恐怕會勸退你。


車禍重傷，當然得掛急診。記者：「發燒肚子痛，就會掛急診？」民眾：「嗯，平常啦，平常急的時候。」記者：「不想等就對了？」民眾：「對，不想等(門診)。」

為了省時，也有人不排門診、掛急診，未來像是牙痛、拉肚子這些小病痛掛急診，健保局打算把診察給付按級分類，醫院恐怕會勸退你，掛門診。民眾：「假如沒有生命危險的話，就(痛)應該還可以忍受的話可以(等)。」

痛，就是其中關鍵，急診檢傷會按照疼痛指數、生命跡象、疾病病因，把病患分5級，最嚴重的第一級像是OHCA，沒有生命跡象，得復甦急救，其次是危急，胸痛就在此類，最常見的發高燒合併任一症狀，是第3級緊急，急性腸胃炎第四級次緊急，但輕微感冒、拉肚子、牙痛，就屬非緊急。

倒是疾病千變萬化，大病通常起因小痛，醫生擔憂健保局做法有顧慮。萬芳醫院急診室主任林樹基：「單純從檢傷去決定病人，是不是輕症還是重症，是有一個潛在的危險性，他可能每次來重複來，都是路倒或是酗酒的情況，但是病患裡面可能有一些，他頭部有外傷但他不知道。」

急診分級只能決定看診順序，真要依級數強制性勸退病患，健保局似乎也得擬好相關配套才行。
