國庫缺錢，財政部盯上火紅的自費醫療院所大查稅！北區國稅局已決定鎖定台北縣、桃園縣等轄區內約三十家自費醫療院所展開「站崗」，查察是否有逃漏稅行為。據悉，這項查稅大規模行動，除了擴及全國五大國稅局與稽徵單位定點「站崗」查稅外，甚至連賦稅署稽核人員也親自「撩下去」。


據悉，今年初各國稅局例行性查察到部分診所短漏報所得逾二十倍、且被國稅局補稅逾千萬元少數個案，因此財政部或各國稅局將知名聯合醫療院所的高昂自費項目，列為今年下半年重點查稅對象。


國稅局總動員 賦稅署也助陣


目前已被財政部盯上的醫療院所自費項目，包括眼科的割雙眼皮及雷射手術，皮膚科的脈衝光、果酸護膚、雷射超音波美白，還有牙科的植牙。


另外，婦產科的做月子中心，專門洗腎的護理之家，還有中醫的減重門診、針灸減肥、三伏貼及轉骨配方等項目，都是查稅重點。


北區國稅局長陳文宗昨天說，由於媒體報導部分自費醫療院所生意興隆，急診掛號到幾百號，看診到凌晨二、三時，但執行業者報稅情況，與實際營收並不太符合，因此決定最近將大規模「駐查」自費醫療院所是否有逃漏稅行為。


國稅局透露，查察逃漏稅手法，考量稽徵人力有限，將會先比對資料，挑出生意好的診所或醫院，然後派員駐點站崗，目的是看生意好到什麼程度，特別的是，稽核人員現在不再只是看帳冊，而是直接調相關人員的銀行帳戶。


以牙醫診所植牙為例，一口牙要價百萬元，全部都是現金給付，且不開收據。且病患是按療程定期付錢，付費方式固定，醫生也定期把錢存入銀行。這些銀行現金，就成了查稅有利證據。


鎖定名醫院所 直調銀行帳戶


會計師表示，健保給付對名醫而言，根本賺不到錢，因為每天限制看病人數的規定，造成申請的健保費相當有限，所以自費看病越來越多，越是名醫收費越高。相反地，健保反而成為名醫的避稅工具。


例如，診所健保實際收入一年一千萬元，但計算要扣除的成本時，是以跟健保局申請的一千兩百萬元健保給付當所得基礎，並先扣除法定允許的七八％執行費用成本約九百三十六萬元，實際所得只剩六十四萬元，比一般上班族賺的少。


業者漏報七年 補稅近五千萬


據悉，目前已被查到醫療院所，漏報所得最高達二十倍，例如實際年賺兩千萬元，但只申報一百萬元。


最近北市國稅局盯上一家知名醫療聯合診所後，從銀行帳戶一口氣追到過去七年漏報的巨額所得，最後被補稅近五千萬元。


此外，北區國稅局下半年查到一家醫療院所短、漏填報自費收入，最後被調增收入金額近兩千萬元案例。
