女性生理年齡 將決定妳的生育能力！

卵巢是決定女性的生育能力最重要的器官，女性受孕率隨年齡下降的原因主要受卵巢功能減弱的影響，因此現代婦女若要完成生育的計畫，建議是在35歲以前，可以較平順的擁有正常的懷孕與生產過程。


女性的年齡是決定生育能力的最重要因素，每一對計畫生育的夫婦，都要將婦女的年齡納入生育計畫中最重要的考量因素，現代女性由於求學與工作的延長，婚姻與期待生育的年齡都嚴重延遲，使得大多數女性都錯過生理上最適合懷孕的時機，不論如何，現代婦女需要了解在35歲以前，完成生育的計畫，可以較平順的擁有正常的懷孕與生產過程。


年齡與受孕能力

晚婚與延遲生育是現代婦女的一個共同現象，這往往使得婦女錯過了生育的黃金年齡，雖然生殖醫學一直在進步，然而在所有不孕的問題中，女性的年齡依然是目前最難克服的因素。一般而言；25歲前婦女受孕率最高，40歲有半數以上不孕，20-24歲有6%不孕，25-29歲有9%不孕，30-34歲有15%不孕，35-39歲有30%不孕，40-44歲有64%，不孕女性年齡是試管嬰兒的懷孕率最重要的關鍵，2005年的全美國人工輔助生育資料中經試管嬰兒治療的嬰兒出生率35歲以是32%，37歲是26%，40歲是16%，而42歲是8%，42歲以上很難懷孕。


卵巢的老化

卵巢是決定女性的生育能力最重要的器官，女性受孕率隨年齡下降的原因主要受卵巢功能減弱的影響，卵巢是卵子的一個儲備庫，卵子由女性出生後就一直在消耗而沒有補充，因此卵巢老化表現在兩個方面：


1.卵子的數量的減少，卵子的數量隨著年齡而明顯減少，卵子的數量由胚胎時期20週約7百萬個，出生時約2百萬個，到青春發育期剩約30萬個，根據研究，女性的卵子數量再約35-36歲以後急速減少，而到更年期前會消耗殆盡。


2.卵子的品質變差；由於卵子是出生後就不再增生，因此女性的年齡等同於卵子的年齡，卵子的老化也就成為卵子品質的最大問題。年紀較大的婦女懷孕有較高染色體異常胎兒的危險，胎兒染色體異常隨母親年齡增加而增加，30歲以前為1/500，30歲為1/270，35歲為1/80，40歲為1/60，45歲為1/20。在產科中將34歲以上的孕婦當作高齡產婦，建議對這些孕婦做全面的羊水染色體檢查，其理由也是染色體異常的機會隨著女性年齡增加而增高。對人類卵子的研究發現35歲以上的卵子其染色體異常的機會明顯增加，利用高解析度的顯微鏡發現在年紀較大的婦女其卵子分裂時的紡垂體通常不正常，在年紀大的婦女其卵子染色體分裂異常的情形遠高於年輕婦女，在胚胎培養的研究也發現年齡40歲以上的婦女其胚胎的染色體分裂異常比率也較年輕婦女為高。若嘗試將人體胚胎培養到囊胚期也發現年紀較大的胚胎停止發育的比率也較年輕的胚胎比率高。


年齡造成的其他不利因素

除了卵巢功能的變差外，年齡帶來的不利生育的因素還有：


1.生殖器官的異常機會增加：如骨盆腔發炎、子宮內膜沾黏、子宮肌瘤或瘜肉也會隨著年齡而增加。


2.年齡增加，流產率增高，2005年全美國的試管嬰兒的流產率由30歲的11%到43歲的43%。


3.年紀較大，生理條件變差，也較難承擔懷孕後的生理變化，高齡產婦懷孕的危險性與生產後遺症遠高於年輕產婦。


懷孕應該及早規劃

在臨床的資料與對人類卵巢的研究都明顯指出女性在35-36歲後生育能力與卵子儲備量都開始急速下降，即使懷孕後流產率也快速上升，40歲以上這種變化更為明顯，因此對於計劃生育的婦女，最好能在35歲以前完成生育的規劃。


至於男性的生育能力，男性35歲以前受孕率最高，50歲受孕率大幅下降但不如女性明顯，男性並特定年齡會喪失生育能力，男性老化其精子活動力及形態變差，但精子濃度影響較小。
