因為常暴躁 所以有躁鬱症？

「醫師！我好像得了躁鬱症，每天都在發脾氣！」一名四十多歲的職業婦女初次來看精神科門診，劈頭就說這句話。我很好奇她為什麼會這樣自我診斷？後來她提到，最近指導小孩課業時，常常發脾氣，她說：「我常常就是在那個時候，躁鬱發作，對兒子大吼大叫。」


近年來，因為媒體廣為宣導，許多民眾對於精神科的診斷名詞都不再陌生了，一般人常用「暴躁」、「容易發脾氣」來描述像這名婦女出現的症狀；而「暴躁」中的這個「躁」字，讓很多人就會直接認定自己是「躁」鬱症了。


生活中，難免會遇到令人生氣的事情，也會偶爾有發脾氣的狀態，這些情緒若是偶爾出現，都屬於正常範圍。然而，就像「發燒」是一個症狀，我們能藉此注意到身體有些不對勁一樣。持續出現暴躁、容易發脾氣的情緒狀態，就是一個「症狀」，出現這樣的指標時，就必須注意到精神、心理狀態可能出了問題。


如同「發燒」這個常見的症狀，從感冒、嚴重的SARS到癌症等很多不同的病症，都可能引起發燒。所以同樣地，持續暴躁或容易發脾氣這個「情緒症狀」，也可能由很多種身心疾病所產生。躁鬱症的確會有易怒（也就是一般所說的「容易生氣」）這個症狀；焦慮症的病患也有可能出現易怒的症狀，所以必須要由專家配合瞭解其他的相關症狀才能確認診斷。


另外，一般人比較容易忽略的是，憂鬱症也會引起易怒這個症狀。最近美國波士頓麻州總醫院發表的一篇研究指出，重度憂鬱的病人，約有一半左右會出現易怒的情緒，也算是相當高的比例。在臨床的經驗中，其他如酒癮、失智症等患者，也都有可能出現易怒的情緒。


以前述這名女性病患來說，在她易怒情緒的背後原因，可能是焦慮症、憂鬱症、躁鬱症等等各種不同的疾病。唯有接受專業的診療，才能仔細追查症狀背後的原因，早期診斷。


易怒的情緒若沒有及早處理，嚴重可能會影響人際關係，包括親密關係、親子關係，或影響工作表現。


此外，也可能引發其他的新問題，例如有些人為了控制脾氣，轉而大量使用酒精，反而造成了酒精濫用或是依賴等問題。早日的診斷，並且早日接受適當的藥物或心理治療，才能夠適時阻止易怒情緒對生活產生的種種不良影響。
