一日洗腎＝終生洗腎？

老陳是一名六十多歲的男性糖尿病患。他因糖尿病腎病變導致腎功能逐日下降，最近開始出現貧血、抽筋、疲勞、噁心、水腫等尿毒症症狀，且症狀一天比一天嚴重，於是求助於腎臟專科醫師。


醫師評估他的狀況後認為，老陳已經到了末期腎病變，必須接受血液透析治療，就是「洗腎」。


老陳接受了血液動靜脈?管手術後，每週3次洗腎，水腫消失，疲勞、抽筋、貧血等症狀改善，且回復到沒有尿毒症時的精神狀態，糖尿病的血糖值也控制得更好。


但令他很煩惱的是，每週都要花3天的時間到洗腎室報到，他聽隔壁阿水嬸聊到，阿水嬸的公公在兩年前因為肺炎敗血症導致腎衰竭，也是在醫院接受洗腎治療，可是洗腎兩週後，就把腎洗好了，到現在都不必洗腎。


老陳的心中不禁燃起一絲希望，洗腎已經快兩個月了，身體的健康狀態也明顯恢復，是否已經把腎洗好了？


洗腎其實是一種替代療法，就是在腎臟沒有功能時，使用人工腎臟透析方式取代腎臟功能，但並非真正的治療，這個取代的時間是暫時？還是一輩子？關鍵在於腎臟壞掉的原因，是屬於急性腎衰竭？還是慢性腎衰竭？比如說：半年前抽血檢查，腎臟功能一切正常，可是因為突發一次嚴重感染（如肺炎、泌尿道感染等），導致敗血症合併急性腎衰竭，很快地腎功能完全喪失，必須接受透析治療，這類病患在經過正確的抗生素治療感染後，腎功能是有機會恢復的，所以這類病患接受洗腎的時間可能在數天，跟數週間，就可以停止。


但在台灣，洗腎患者有一大族群是屬於慢性腎病變，如糖尿病、高血壓跟慢性腎絲球腎炎所導致的慢性腎衰竭，這類患者的腎功能是逐年逐月衰退，腎臟逐日萎縮硬化，喪失功能，到最後走向末期腎病變，而要洗腎。


像這類患者的腎臟並無法恢復，必須終身洗腎，除非他得到一顆新的腎臟（也就是換腎治療），才能停止規則透析治療。


洗腎並不是一種直接治療腎臟的方式，只是一種腎臟替代療法，所以不會把腎臟洗好，之所以有洗腎把腎洗好的案例，是因為其腎衰竭屬於急性腎衰竭，在及時適切的治療後，恢復腎功能，並非把腎洗好。
