小心抗藥性流感大流行

　近日H5N1禽流感在中國大陸現蹤、抗藥性流感在世界各地出現，這些都是我們必須正視的警訊。

　二○○三年以後H5N1禽流感席捲東南亞，只有台灣與菲律賓完全免疫，兩地的家禽與野鳥都未曾分離過這種病毒。筆者問過菲律賓醫師，他們似乎沒什麼特別的檢疫作為，但雞肉是當地最便宜的肉類，所以他們到處養雞。筆者認為菲律賓的雞隻自給自足、不須進口，台灣則禁止大陸農產品進口，這或許是台灣與菲律賓能免於H5N1禽流感侵擾的主要因素。在台灣與大陸開始三通之後，特別需要注意走私農產品的查緝；否則，H5N1禽流感終有一天會登陸台灣。

　流感病毒的抗藥性是另一重要議題。一九九七年H5N1禽流感在香港出現時，金剛胺是有效的治療藥物。沉寂數年後，H5N1禽流感於二○○三年在東南亞再度浮現，它們突然對金剛胺都有抗藥性。

　病毒發生突變以適應藥物，常必須犧牲一些原有的構造或功能，所以通常只有在碰到大量藥物的情形下，病毒才會突變而產生抗藥性。二○○三年H5N1禽流感病毒莫名其妙地出現金剛胺抗藥性，表示一定有人濫用藥物。過去外電即曾經報導，大陸對雞隻大量使用金剛胺。現在，金剛胺也對H3N2人流感幾乎無效。筆者認為禽類身上的金剛胺終究會被吃入人體，這跟人流感的抗藥性脫離不了關係。

　最近，被視為流感大流行最後一道防線的克流感也出問題了。今年在美國與日本流行的H1N1人流感病毒，幾乎都對克流感有抗藥性。此外，亞洲地區分離出來的H5N1禽流感，對克流感也有感受性愈來愈差的趨勢。有學者認為這是無法解釋的自然突變，但自然突變之說完全違反物競天擇的原理。

　日本為了儲備大量克流感以因應未來的世界大流行，每年消耗掉全人口十分之一數量的克流感，這是促使流感病毒產生抗藥性的助力之一。但日本濫用克流感，理應不會影響到H5N1禽流感，因為人吃的藥物不會隨便跑到家禽身上。世人最應擔心的，是有沒有人在飼料內偷加克流感？

　為了因應流感大流行的危機，除了加強檢疫之外，我們呼籲大家一起重視克流感抗藥性的問題，無論人類或家禽都不應該濫用克流感，相關國家更應加強查核飼料是否任意添加藥物。未來如果我們重返世界衛生組織議場，這是可以提出的重要議題。國內衛生單位也應注意，一旦克流感無效之後，將來的防疫可能必須倚重瑞樂沙等其他抗流感藥物。
