醫療收據不清不楚 醫改會做評比

【記者江昭青報導】時機歹歹，醫改會發現，不少民眾投訴醫療診所的「自費項目」增加了，經進一步了解，發現許多診所的醫療收據根本寫不清楚，民眾自費項目也付得「莫名其妙」，而醫改會針對各地方政府進行的調查結果也顯示，僅有宜蘭、新竹、苗栗、金門、基隆五縣市衛生單位善盡管理責任，為督促醫療院所落實醫療收據需符合規定，民眾也可向醫改會檢舉。
台灣醫療改革基金會，今天公布2008年各縣市對於民眾就醫權益的努力成果，分別就「合格醫療收據」、「醫療收費標準」、「推廣兒童用藥」、「老人藥事服務」、「醫療糾紛調處」等項目做評斷。
醫改會董事長張苙雲表示，各縣市雖然沒有在上述五項中，都做得十分完美，但至少都有一項很用心；其中桃園縣、台北縣、屏東縣都有四項達到標準，在維護民眾就醫上有不錯的作為，表現最佳。
張苙雲舉例，屏東縣醫療或行政資源不是最豐富，但在兒童用藥、醫療收據等方面，都督促醫院滿足民眾知的需求；桃園縣光是為了兒童用藥問題，就舉辦了100場講座宣傳，都顯示非常用心。
但張苙雲也說，近來可能因為不景氣，民眾投訴醫療院所「自費增加」的問題變多了，其實主要是因為醫院的醫療收據，註明不夠清楚，讓民眾不知道哪些自費？哪些是健保給付。她表示，衛生署針對醫療收據的表格有規範，但部分醫療院所仍未遵守，要靠地方衛生單位督促改進，但目前只有宜蘭縣、新竹市、苗栗縣、金門縣、基隆市因官方要求，醫療院所的單據符合規範，標示很清楚。
醫改會人員也透露，目前大醫院在醫療單據上，多數符合規定，民眾比較常反應的是，一般小診所的單據說明不清，使他們不知道自己吃的藥竟然要自費。此外，不少民眾反應，住院或手術使用的止痛藥，有的要自費、有的不用，但在醫療單據上，連藥名都沒寫清楚，讓患者想申訴，都說不出藥名。這些都要官方持續督促改進。
至於健保費是否調漲問題，張苙雲說，健保給付中，有四分之一都在藥費上，而四分之一的醫療糾紛也出在用藥不當；為何醫療資源有那麼多用在藥品上，藥價是否仍存在黑洞？健保局都要先解決，真的努力處理藥價問題後，再來談健保漲價，人民才能接受。
