這是Fosamax最可怕的併發症
而且無法可救!!

Cancer patients with bone metastasis & pain 的 病人常用 Bisphosphonate.
主要是它能抑制 osteoclast 作用

關於副作用 osteonecrosis of jaw (ONJ), Bamias A 等人 在 J Clin Onco 2005, Dec 1 的報告:

研究了252 例 打 IV bisphosphonate 的cases(1997,1~2003,7), 發現
17 pts (6.7%) 產生 ONJ: 11 of 111 (9.9%) with multiple myeloma, 2 of 70 (2.9%) with breast ca, 3 of 46 (6.5%) with prostate ca, and 1 of 25 (4%) with other neoplasms (P = .289).

也發現發生ONJ跟使用時間長短有關: 有 ONJ 的 treatment cycle 和使用期間與沒 ONJ 的比: 35 vs 15 infusion (p<.001); 39.3 vs 19 months (p= .001)

The incidence of ONJ increased with time to exposure from 1.5% among patients treated for 4 to 12 months to 7.7% for treatment of 37 to 48 months.

使用zoledronic acid (Zolmeta) 比使用 pamidronate (Aredia) 的發生 ONJ 機會高( p<.001)

而根據 Pazianas M. 等人在 Clin Ther 2007, Aug 對於osteoporosis 病人使用 biphosphonate 導致的 ONJ, 作了分析:
Review 了由 1966~2006/9 的 相關報告, 共11篇報告了26個因治療骨鬆,使用 biphosphonate後產生ONJ的case.
主要是在 mandible (16 pts),maxilla (6 pts).
知道年紀的23人, 其中 78% 的年紀 >or=60 years, only 3(13%) were men. 而其中15人有 invasive dental treatment 病史,12 (80%) 發生的 ONJ 是在dental surgery or experienced dental trauma處.
其中只有10人能知道接受 bisphosphonate的治療時間, 但看來似乎和ONJ 發生無相關.

看起來是bisphosphonate是蠻安全的. 當然是不是低估了危險性?
(有些case是不會報在paper上的)

還是要小心點好.
