美國退休人員明年將面對雙重衝擊：社會安全福利不調漲，聯邦醫療保險（Medicare）一些保費卻將暴漲52%。


Medicare保費提高，將只影響沒有「不受損害」（hold harmless）條款保障的30%受益人。這種條款禁止任何會導致社安福利實質減少的保費上漲，不適用對象只有個人年所得達到8萬5000元（美元，下同），夫妻達到17萬元以上的富人，以及參加Medicare第一年的人。



但是，國家廣播公司報導說，保費漲幅之劇，強烈顯示近年來健保費用溫和上漲之勢即將結束。



2016年的社安金和Medicare保費要到入秋才定案，不過社安署最近提出的年度報告，預計社安金明年不會有任何生活費用調整（COLA） 。



健保開支膨脹率近年來比較溫和，從2010到2014年Medicare每年整體開支只平均提高4.1%，遠低於2000至2010年的9%成長率，雖然參加的受益人不斷增加。



但是，年度報告指出，費用壓力加強顯示明年Medicare保費將暴漲。例如，Medicare Part B（門診服務）的月費三年來一直維持104.90元，可是明年沒有「不受損害」條款保護的人，月費將暴漲52%，達到159.30元。



受影響的將包括已參加Medicare可是尚未申領社安金的人，明年才參加Medicare的人，以及由州政府的醫療補助計畫（Medicaid）支付保費的低收入受益人。



高收入退休者也面對保費大漲，而他們原已為Medicare Part B和Part D（處方藥福利）支付較高的保費。例如，根據所得高低，他們今年的Part B月費在146.9元至335.7元。



摩根資產管理公司的研究指出，2014年65歲的人中位健保花費為4400 元，而預計未來20年每年將增加6.1%，85歲時達到1萬7000元。這種開支包括Medicare Part B、Part D和輔助醫療照顧保險（Medigap）的保費，各種自付額，以及眼科和牙科服務。
