老人跌倒複雜 會診找原因

成大醫院「跌倒預防門診」開診一年，發現老人跌倒的原因很多，且常有多重因素交互影響，成大醫院老年科醫師白惠文說，要確實找出老人跌倒病因，需要各科醫師會診，必要時居家訪視，才能對症下藥。

白惠文說，老人跌倒對健康危害相當大，再發率非常高，醫療負擔也相當昂貴，如果造成骨折長期臥床，會更快衰老，尤其髖關節骨折老人，1年內的死亡率高達3成，不可不防。

成大醫學去年7月成立「跌倒預防門診」，結合醫學、護理、藥學、物理治療、職能治療養醫，為有跌倒病史及高危險老人擬定治療計畫，除了醫療、復健，也包含住家環境的改善。白惠文說，老人跌倒的原因複雜，很多是潛在因素，從腳沒有力氣、步態不穩、頭暈，到有疾病在身、營養不良，甚至藥物影響、環境因素，都會造成老人跌倒。

白惠文指出，早期巴金森氏症患者會有小碎步、走路前傾，如果尚未被診斷出來，就被當成不明原因跌倒。營養不良體力變差，也可能導致跌倒，問題出在飲食不均衡或消化系統疾病。憂鬱症病人足不出戶，久而久之肢體無力、體力衰退，走路功能都會退化，容易導致跌倒發生。

其他還有老人家視力不好，看不清楚門檻、階梯、地面物品。聽力不好，沒聽到家人的提醒，都很容易跌倒、或在濕滑的地方滑倒。住家照明不足，廁所浴室沒有扶手，也是潛在因素。要預防跌倒，就得先加強廁所的照明、扶手等安全設備。
