衛生署「二代健保總檢討小組」召集人、慈濟大學教授葉金川昨天表示，補充保費從元旦上路迄今，徵收作業確實不便民，小組已提出檢討，並討論改採年度結算的可行性。

今年起，補充保費新制上路，從健保局日前公布元月約進帳十八億元，與原先預期差不多。

不過，葉金川也表示，最近「二代健保總檢討小組」首次開會，與會者達到多項共識，認為未來補充保費的改革，必須要符合「簡便、公平、費基不流失」三點原則。

葉金川指出，目前補充保費最受人詬病之處，在於徵收作業過於繁複，民眾若有兼職所得、租金、利息等收入，只要每筆超過五千元，或是獎金大於投保金額的四倍，就要扣補充保費，扣費義務人還要下載程式及繳費等程序，徒增加許多作業成本。

他認為，為簡化作業及降低行政成本，未來補充保費應可改為年度結算方式，只要向國稅局取得民眾報稅資料，掌握每人非薪資所得的年度收入，並模擬試算補充保費的起徵點，不必每筆都計算。

他說，現行健保法在補充保費是採就源扣繳，與年度結算的方式並不互斥，他希望不修法或法律微調，就能執行；但長期目標仍是在三、五年內朝更完美的「三代健保」修法。

景文科技大學教授朱澤民說，「真理總是寂寞的」，當初補充保費上路前，他就曾警告如果每筆收入都要扣，勢將「勞民傷財」。

朱澤民說，若改成年底依總所得結算，就不會有定存拆單潮或選擇性計費不公的情況。

葉金川表示，由於補充保費的繳交會有「季節性」高低之分，因此需待今年五到七月股利、股息的繳交情況，整個檢討報告要半年後才會出爐。

中視新聞》二代健保補充保費上路，一月就進帳80億，但是因為手續太麻煩，健保局考慮，將改用年度結算的方式徵收。雖然保費多繳一點點，但是台灣健保實在太好，每年有50國的代表前來取經，很多僑胞也專程回台看醫生。台大小兒科醫師李秉穎就說，台灣老百姓不要生在福中不知福，浪費健保就是害了大家。
