衛署：長照保險降服務障礙

多名民間團體代表今天對長期照護服務的財源憂心。行政院衛生署參事曲同光說，長照保險好處是集合眾人力量分擔，讓服務更沒障礙，若採社會福利模式，牽涉國家稅收負擔能力。

中國國民黨籍立法委員蔡錦隆、楊玉欣今天在立法院舉行「長期照護保險法草案」公聽會。蔡錦隆表示，國內需要長照人口快速增加，長照計畫應儘速建構可長可久的服務網和保險制度。

曲同光會中報告表示，長照保險籌備已久，目前規劃保費採政府部分補助、雇主及個人部分負擔，類似現在健保收費方式，至於保費多寡、費率，希望3年檢討一次。

多名與會民團代表對長照保險制度、經費憂心。台灣社區醫院協會代表張克士會中表示，現在醫療環境因為資源少，已經走到懸崖邊，很多醫師不願救人只願救美。長照必須長期維繫，不可能賠錢就關門，須審慎思考是否循健保模式。

與會的台灣社區醫院協會代表朱益宏也說，長照的財務思維類似健保，若未來長照財源不足，面臨該不該漲價問題，就會跟現在健保調整保費的窘境一樣。

曲同光會後受訪表示，採用保險的好處是集合眾人力量共同分擔，降低接受服務的障礙。如果不用保險，改採社會福利，牽涉國家有沒有這麼多稅收支援，若沒有，可能產生服務不足、有需要的人必須多花錢狀況，如何取捨是兩難，也必須思考若不採保險，如何讓長照有永續經營的本錢。

他說，很多人談到健保導致醫療問題，但今天發生的醫療現象和問題「如果沒有健保就不會發生嗎？」，若把過多缺失歸咎健保，可能太武斷，也讓健保承擔太多責任。

多名民團代表也關心身心障礙者、老人、幼童在長照服務體系定位問題，如小孩繳了保費可能要很久才能接受服務。

曲同光受訪時說，長照服務的對象是失能者，評估失能狀況提供所需協助，若身心障礙者沒有失能，就不符合服務資格。至於年輕人、小孩也有可能失能，屬風險問題，若不降低納保年齡，這些人就無法接受長照服務。

至於長照保險推動時機，曲同光說，很多人擔心有保險，但服務跟不上，推動時機端看長照體系整備狀況來考量。
